卡前列素氨丁三醇对预防剖宫产产后出血的疗效及对凝血功能、血流动力学的影响

Effect of Carprost Trometamol on Postpartum Hemorrhage after Cesarean Section and Its Effect on Coagulation Function and Hemodynamics

目的:观察卡前列素氨丁三醇对预防剖宫产产后出血的疗效及对凝血功能、血流动力学的影响。方法:以某院2016年8月~2017年6月进行剖宫产术的产妇100例为研究对象,按照随机数字表法随机分为观察组和对照组各50例。对照组给予缩宫素治疗方案,观察组给予缩宫素联合卡前列素氨丁三醇治疗,比较两组术后出血量、产后出血发生率及凝血功能、血流动力学水平的变化。结果:观察组术后2h出血量(241.30±72.11)mL、术后24h出血量(321.75±89.51)mL均显著低于对照组,差异有统计学意义(P<0.05);观察组产后出血发生率6%显著低于对照组产后出血发生率14%,差异有统计学意义(P<0.05);术后观察组凝血功能指标活化部分凝血活酶时间(APTT)水平(41.67±8.23)s、血浆凝血酶时间(TT)水平(15.82±4.51)s、纤维蛋白原(Fib)(3.02±1.06)g/mL显著低于对照组相应水平,差异有统计学意义(P<0.05)。结论:卡前列素氨丁三醇预防剖宫产产后出血有显著疗效,且对改善血液高凝状态、维持血流动力学稳定状态有明显促进作用,具有临床应用价值。

Objective：To observe the effect of carprost trometamol on postpartum hemorrhage after cesarean section and its effect on coagulation function and hemodynamics.Methods：100 cases of patients with cesarean section treated in a hospital from August 2016 to June 2017 were selected as the research object and randomly divided into observation group and control group,with 50 cases in each group.The control group was given oxytocin treatment,and the observation group was treated with oxytocin combined with carboprost trometamolm,then the postoperative bleeding volume,postpartum hemorrhage rate,coagulation function in the two groups were compared.Results：After operation,the bleeding volume of 2 hand 24 hafter operation of the observation group were（241.30±72.11）mL and（321.75±89.51）mL,which were significantly lower than those of the control group,and the difference was statistically significant（P〈0.05）.The incidence of postpartum hemorrhage in the observation group（6%）was significantly lower than that in the control group（14%）,the difference was statistically significant（P〈0.05）.After surgery,the levels of activated partial thromboplastin time（APTT）[（41.67 ±8.23）s],plasma thrombin time（TTT）[（15.82 ±4.51）s]and plasma thrombin time（TTT）[（3.02 ±1.06）g/mL]in the observation group were significantly lower than those in the control group,the difference was statistically significant（P〈0.05）.Conclusion：The prevention of postpartum hemorrhage by caesarean section with carprost trometamol has significant curative effect,which can improve blood hypercoagulability and maintain hemodynamic stability,and has clinical application value.
