VCAM-1和AOPP与妊娠期糖尿病患者母婴结局的关系研究 被引量:17

摘要 目的研究血管细胞黏附分子-1(VCAM-1)和晚期氧化蛋白产物(AOPP)与妊娠期糖尿病(GDM)患者母婴结局的关系。方法纳入125例GDM患者(GDM组)和108例健康孕妇(对照组),检测GDM相关指标、母血和脐带血VCAM-1及AOPP含量,分析VCAM-1和AOPP与母婴结局的关系。结果 GDM组空腹血糖、糖化血红蛋白、低密度脂蛋白、载脂蛋白B含量显著高于对照组,高密度脂蛋白、载脂蛋白A显著低于对照组(P<0.05);GDM组母血和脐带血的VACM-1和AOPP含量均显著高于对照组(P<0.05);GDM组妊娠成功率95.20%,妊娠丢失率4.80%,妊娠成功者母血VCAM-1和AOPP水平显著低于丢失者(P<0.05);GDM组中,发生产科并发症者(61.60%)的母血VCAM-1和AOPP水平均显著高于无并发症者(P<0.05);GDM组125例新生儿中,巨大儿32%,早产儿13.6%,低体重儿6.4%,新生儿低血糖4.8%,新生儿窒息3.2%,死胎1.6%,其脐带血VCAM-1和AOPP水平均显著高于足月正常新生儿(P<0.05)。结论 GDM孕妇血清和脐带血VCAM-1和AOPP水平显著高于正常孕妇,其水平升高与母婴不良结局密切相关。 Objective To study the correlation between VCAM-1/AOPP levels and maternal and neonatal outcomes of GDM patients. Methods The research involved 125 GDM patients("the GDM group") and 108 healthy pregnant women("the control group"). GDM-related indicators and VCAM-1 and AOPP levels in maternal blood and cord blood were tested to analyze the correlation between VCAM-1/AOPP and maternal and neonatal outcomes. Results Indictors including FBG, glycated hemoglobin,LDL and content of apolipoprotein B in the GDM group were significantly higher than those in the control group while HDL and apolipoprotein A were significantly lower than those in the control group(P〈0.05). VACM-1 and AOPP levels in maternal blood and cord blood in the GDM group were significantly higher than those in the control group(P〈0.05); the success rate of pregnancy and the loss rate of pregnancy in the GDM group were 95.20% and 4.80% separately; the VCAM-1 and AOPP levels in maternal blood of the women who were successfully pregnant were significantly lower than those of the women having failed in pregnancies( P〈0.05); in the GDM group, the VCAM-1 and AOPP levels in maternal blood of those with obstetric complications(61.60%) were significantly higher than those without obstetric complications(P〈0.05); in the 125 newborns in the GDM group, there were macrosomia(32%),premature(13.6%), infants of low birth weight(6.4%), neonatal hypoglycemia infants(4.8%), neonatal asphyxia(3.2%) and stillbirth(1.6%), of which the VCAM-1 and AOPP levels in cord blood were significantly higher than those of normal newborns(P〈0.05).Conclusion VCAM-1 and AOPP levels in serum and cord blood of GDM pregnant women are significantly higher than those of normal pregnant women. The rise of VCAM-1 and AOPP levels is closely related to adverse maternal and neonatal outcomes.
