激光治疗BPH致膀胱梗阻的效果及对性功能的影响

Effects of laser treatment for BPH-induced obstruction of bladder and impacts on sexual function

目的探讨激光前列腺汽化术治疗良性前列腺增生(BPH)致膀胱出口梗阻的临床疗效及其对患者性功能的影响。方法将医院收治的84例BPH致膀胱出口梗阻患者作为研究对象,根据治疗方式的不同分成两组,每组42例。对照组采用常规电切术治疗,观察组采用经尿道120 W绿激光前列腺汽化术治疗。比较两组治疗后前列腺症状评分情况、最大尿流率、膀胱残余尿量以及性功能情况。结果治疗后,观察组前列腺症状(IPSS)评分和膀胱残余尿量低于对照组,最大尿流率大于对照组,阴茎勃起功能障碍、射精异常和射精疼痛构成比均低于对照组(P<0.05)。结论采用经尿道120 W绿激光前列腺汽化术治疗BPH致膀胱出口梗阻的临床效果确切,能够显著改善患者梗阻情况,同时对性功能影响较小,有助于提高患者生活质量。

Objective To explore the clinical effects of the photoselective vaporization of prostate treatment for BPH-induced obstruction of bladder and the impacts on sexual function of the patients. Methods A total of 84 patients who had been suffering from BPH-induced obstruction of bladder and had been admitted into the hospital were taken as the research subjects and divided into two groups according to the treatment methods（n=42, respectively）. The patients in the control group received conventional electrocision, while the patients in the observation group received urinary treatment with the 120 W green-laser photoselective vaporization of prostate method. The IPSS assessment, maximum flow rate, bladder residual urine volume and sexual function were compared between the two groups. Results With the treatment, the IPSS assessment and bladder residual urine volume of the observation group were lower than those in the control group; the maximum flow rate was higher than that in the control group; the erectile dysfunction, ejaculation anomaly and painful ejaculation constituent ratio were lower than those in the control group（P〈0.05）. Conclusion The clinical effects of the urethra 120 W green-laser photoselective vaporization of prostate treatment for BPHinduced obstruction of bladder and the impacts on sexual function of the patient are conclusive. The treatment can significantly improve obstruction and has minor adverse impact on the sexual function of the patients, which is favorable for improving the quality of life of the patients.
