鼻中隔端端移植在鼻尖成形中的应用

Application of end-to-end transplantation in the tip rhinoplasty

目的通过鼻中隔移植物在鼻中隔尾侧端的进行端端移植术,建立稳定的鼻尖支撑结构,并使鼻中隔在长度和高度两个方向上得到延伸,以获得美观持久的鼻尖形态。方法使用鼻中隔移植物,使鼻中隔前脚在及尾侧缘的长度和高得到延伸,同时将保留的L形支架进行加强,再将下外侧软骨穹窿,下外侧软骨内侧脚延伸的中隔移植物缝合固定,从而形成稳定的鼻尖支撑结构。结果 39例受术者随访3-18个月,其中4例出现鼻尖突度略有下降,但形态良好,2例鼻中隔偏曲的患者,在2个月后再次出现偏斜。其余32例均鼻尖形态良好。结论利用鼻中隔移植物行鼻中隔端端的移植抬高和延长鼻尖,鼻尖形态稳定,并且能够减少鼻中隔软骨的浪费。是一种有效的方法。

Objective To undergo the end-to-end transplantation with nasal septum grafts, establish the stable nasal tip structure, extend the length and height of nasal septum and reconstruct the esthetic nasal tip shape. Methods The nasal septum grafts were applied to extend the length and height of anterior and posterior nasal septum, strengthen the L-shape support, suture and fix the extended nasal septum grafts in the lower lateral cartilage fornix and lower lateral cartilage medial crus and establish the stable tip structure. Results 39 patients were followed with 3-18 m including 4 cases of slightly declining nasal tip salient with good shape, 2 cases of nasal septum deviation and recurrent deviation after 2 m and 32 good shape cases. Conclusion During the elevation and extension of the apex nasi, the end-to-end transplantation with nasal septum grafts can effectively stabilize the nasal tip shape and reduce the injury of septal cartilage.
