全腔镜与腔镜辅助直肠癌前切除临床对比研究

A comparative study of total endoscopic and laparoscopic assisted anterior resection for rectal cancer

目的:比较全腔镜下直肠外翻拖出式直肠癌前切除术与腔镜辅助直肠癌前切除术的临床疗效。方法:选取2010年5月至2012年5月我院诊治的76例直肠癌患者,依据数字表法随机分组,观察组(A组,n=38)采用全腔镜下直肠外翻拖出式直肠癌前切除术,对照组(B组,n=38)采用腔镜辅助直肠癌前切除术,随访5年,对两组直肠癌患者的术中、术后指标以及生存率进行统计学分析。结果:两组手术时间、术中出血量、切除标本长度、清扫淋巴结数目、5年生存率等差异无统计学意义;A组患者排气时间、进食时间、住院时间、切口长度、切口并发症均较B组明显减少,P值小于0.05、差异有统计学意义。结论:全腔镜下直肠外翻拖出式直肠癌前切除术安全、有效,具有创伤痛苦小、最佳美容、并发症少、恢复快等优点。

Objective： To study the clinical efficacy of total endoscopic anterior resection for rectal cancer by pull-through operation by comparing laparoscopic assisted anterior resection for rectal cancer. Methods： From May 2010 to May 2012 76 cases of rectal cancer patients in our hospital were selected and grouped randomly according to the digital table. There are two groups： The study group（ A group,n = 38） of total endoscopic anterior resection for rectal cancer by pull-through operation and the control group（ B group,n = 38） of laparoscopic assisted anterior resection for rectal cancer. The intraoperative and postoperative data and five-year survival rate were compared between the two groups. Results： There were no significant differences in the operation time,blood loss,length of the specimens,number of lymph nodes and five-year survival rate between the two groups. The difference between the study group and control group was statistically significant in the exhaust time,eating time,hospital stays,length of the incisions and incision complications. Conclusion： The total endoscopic anterior resection for rectal cancer by pull-through operation is effective,safe and feasible. It has the advantages of less trauma,better cosmetic,less incision complications and quicker recovery.
