去骨瓣减压术对对冲性颅脑外伤颅内压和预后影响的Meta分析

Effect of bilateral decompressive craniectomy on intracranial pressure and prognosis of severe traumatic brain injury:a Meta-analysis

目的通过Meta分析的方法探讨国内应用双侧去骨瓣减压术治疗对冲性颅脑外伤的病例,分析此手术方式对患者颅内压和预后的影响。方法通过关键词检索中文期刊数据库:中国知网(CNKI)、万方数据库、中国生物医学数据库(CBM)。由两位工作人员筛选符合纳入标准的文献,并提取本研究所需的数据,用Stata 14.0软件对提取的数据进行分析。结果双侧去骨瓣减压术的有效率差值比(RD)为0.22,其95%置信区间(CI)为0.16~0.28,病死率的相对风险比(RR)为0.48,其95%CI为0.35~0.67。结论双侧去骨瓣减压术能够显著性增加治疗有效率和降低术后病死率,有效改善手术治疗的预后。

Objective The effects of bilateral decompressive craniectomy after traumatic brain injury on the intracranial pressure and the prognosis were investigated by using the Meta-analysis. Methods Chinese database,such as,CNKI,Wanfang database and Chinese Biomedical Database（ CBM） were searched for the key words.Filtering the articles was performed by two staff members with the inclusion and exclusion criteria,and then extracted the data in this study as required. Stata 14. 0 Software was used to analyze the extracted data. Results Bilateral decompressive craniectomy effectiveness risk difference（RD） was 0. 22 with 95% confidence interval（CI）（0. 16 ~0. 28）,and the mortality risk ratio（ RR） was 0. 48 with 95% CI（0. 35 ~ 0. 67）. Conclusion Bilateral decompressive craniectomy can significantly increase the efficiency of surgery,reduce the post-operative mortality and improve the prognosis of surgical treatment.
