摘要
目的探讨间歇充气加压装置配合低分子肝素对老年髋骨骨折患者深静脉血栓形成的预防价值。方法选取2015年7月至2017年8月间我院收治的80例老年髋骨骨折患者,随机分为两组各40例。对照组患者予以1 m L低分子肝素进行治疗,1次/d,连续用药10 d;观察组患者在对照组的基础上,配合予以间歇充气加压装置进行治疗,10 h/d,连续应用10 d。结果观察组的深静脉血栓发生率为5.0%,显著低于对照组的17.5%(P<0.05)。两组的术中出血量、术后引流量及术后血小板计数比较,差异无统计学意义(P>0.05)。结论间歇充气加压装置配合低分子肝素在老年髋骨骨折患者中的应用,可以显著降低深静脉血栓形成的发生率,同时不会增加术中的出血量、术后引流量及并发症发生风险,具有较高的安全性,值得推广应用。
Objective To explore the value of intermittent pneumatic compression device combined with low molecular weight heparin (LMWH) in the prevention of deep venous thrombosis in elderly patients with hip fracture. Methods 80 cases of elderly patients with hip fracture admitted to our hospital from July 2015 to August 2017 were selected and randomly divided into two groups, with 40 cases in each group. Patients in the control group were treated with 1 mL LMWH once per day for 10 days continuously. Patients in the observation group were treated with intermittent pneumatic compression device 10 hours per day for 10 days on the basis of control group. Results The incidence of deep venous thrombosis of the observation group was 5.0%, significantly lower than 17.5% of the control group (P〈0.05). No statistical difference was found between two groups in the amount ofintraoperative blood loss, postoperative drainage volume or postoperative platelet count (P 〉0.05). Conclusions Intermittent pneumatic compression device combined with LMWH applied in elderly patients with hip fracture can significantly reduce the incidence of deep venous thrombosis, without the increase of intraoperative blood loss, postoperative drainage volume or risk of complications, which shows high safety, and is worthy of promotion and application.
