五皮饮加减联合奥曲肽治疗肝硬化顽固性腹水的临床效果研究

Study on the Clinical Effect of Modified Wupi Decoction Combined with Octreotide in the Treatment of Intractable Ascites of Liver Cirrhosis

目的探讨五皮饮加减联合奥曲肽治疗肝硬化顽固性腹水的临床效果。方法收集我院2013年1月至2016年1月收治的肝硬化顽固性腹水患者46例,随机分为观察组和对照组各23例。两组患者均给予内科综合治疗和奥曲肽皮下注射治疗,在此基础上,观察组患者给予五皮饮进行加减治疗,观察两组患者的临床治疗效果。结果治疗后,两组患者的症状评分、24小时尿量、腹围和体重各项指标显著优于治疗前,且观察组的各项指标均显著优于对照组,差异有统计学意义(均P<0.05)。观察组的临床治疗总有效率显著高于对照组(95.7%vs.69.6%),2个月后腹水复发率显著低于对照组(8.70%vs.34.8%),差异有统计学意义(P<0.05)。结论五皮饮加减联合奥曲肽治疗肝硬化顽固性腹水的疗效显著,值得临床应用推广。

Objective To explore the clinical effect of modified Wupi decoction combined with octreotide in the treatment of intractable ascites of liver cirrhosis. Methods 46 cases of patients with intractable ascites of liver cirrhosis admitted to our hospital from January 2013 to January 2016 were selected and randomly divided into the observation group and the control group, with 23 cases in each group. Both groups received comprehensive treatment of internal medicine and subcutaneous injection of octreotide; in addition, the observation group was treated with modified Wupi decoction. The clinical curative effects of two groups were compared. Results After treatment, both groups had significantly improved symptom score, 24-hour urine volume, abdominal circumference and body weight, and the indicators of observation group were significantly better than those of control group （P〈0.05）. The total effective rate of clinical treatment of observation group was 95.7%, significantly higher than 69.6% of control group （P〈0.05）. The recurrence rate of ascites of observation group was 8.70%, significantly lower than 34.8% of control group （P 〈0.05）. Conclusions Modified Wupi decoction combined with octreotide in the treatment of intractable ascites of liver cirrhosis has significant efficacy, which is worthy of clinical promotion and application.
