快速康复外科理念在老年髋关节置换患者围术期的应用

The Application of Fast-Track Surgery Concept During Perioperative Period in Elderly Patients with Hip Arthroplasty

目的探讨老年髋关节置换患者围术期应用快速康复外科理念的临床效果。方法选取我院98例老年髋关节置换患者,按照入院先后顺序分为观察组和对照组各49例,对照组实施传统康复,观察组在对照组基础上予以快速康复外科理念,比较两组的康复效果。结果两组术前的HAMA、VAS、康复信心评分比较,差异无统计学意义(P>0.05);观察组术后的HAMA、VAS、康复信心评分均显著优于对照组(P<0.05)。观察组的用药时间、下床活动时间、术后住院时间、住院费用均显著少于对照组(P<0.05)。观察组的术后并发症发生率为4.1%,显著低于对照组的16.3%(P<0.05)。结论对老年髋关节置换患者实施围术期快速康复外科理念,能够有效缓解患者的不良情绪,改善疼痛程度,缩短术后下床活动时间和住院时间,减少术后并发症。

Objective To explore the clinical effect of application of fast-track surgery concept during perioperative period in elderly patients with hip arthroplasty. Methods 98 eases of elderly patients with hip arthroplasty in our hospital were selected and divided into two groups according to the admission order, with 49 cases in each group. The control group received traditional rehabilitation, while the observation group received fast-track surgery concept on the basis of traditional rehabilitation. The rehabilitation effects of two groups were compared. Results Before surgery, no statistical difference was found between two groups in the HAMA, VAS and rehabilitation confidence scores （P 〉0.05）; After surgery, the HAMA, VAS and rehabilitation confidence scores of the observation group were significantly better than those of the control group （P〈0.05）. The medication duration, out-of-bed activity time, postoperative hospitalization time and hospitalization cost of the observation group were significantly lower than those of the control group （P 〈0.05）. The incidence of postoperative complications of the observation group was 4.1%, significantly lower than 16.3 % of the control group （P〈0.05）. Conclusions Fast-track surgery concept applied in elderly patients with hip arthroplasty during perioperative period can effectively relive patients＇ bad mood, improve the pain degree, shorten the postoperative out-of-bed activity time and hospitalization time, and reduce the postoperative complications.
