血清同型半胱氨酸、维生素B12和凝血功能与先兆流产及其妊娠结局的关系

Relationship of Serum Homocysteine, Vitamin B12, Coagulation Function with Threatened Abortion and Pregnancy Outcome

目的探讨血清同型半胱氨酸(Hcy)、维生素B12(VB12)和凝血功能与先兆流产及其妊娠结局的关系。方法选取2017年4月至2017年11月在我院确诊为先兆流产的孕妇80例作为先兆流产组,另选择同期在我院进行产前检查的健康孕妇80例作为正常妊娠组。收集两组的临床资料,并检测其血清Hcy、VB12、D-D/R水平,分析血清Hcy、VB12、D-D/R水平与妊娠结局的关系。结果正常妊娠组的血清Hcy和D-D/R水平显著低于先兆流产组(P<0.05),VB12水平显著高于先兆流产组(P<0.05)。Spearman相关分析显示,血清Hcy、D-D/R水平与难免流产呈显著正相关(rs=0.824、0.819,P<0.05),血清VB12水平与难免流产呈负相关(rs=-0.633,P<0.05);血清Hcy、D-D/R水平与保胎成功呈负相关(rs=-0.687、-0.614,P<0.05),血清VB12水平与保胎成功呈正相关(rs=0.642,P<0.05)。结论联合检测血清Hcy、VB12、D-D/R水平,对评估先兆流产及其妊娠结局有一定的临床意义。

Objective To explore the relationship of serum homocysteine （Hcy）, vitamin B12 （VB12）, coagulation function with threatened abortion and pregnancy outcome. Methods A total of 80 cases of pregnant women diagnosed as threatened abortion in our hospital fi＇om April 2017 to November 2017 were selected as the threatened abortion group; 80 cases of healthy pregnant women with prenatal examination in our hospital at the same period were selected as the normal pregnancy group. The clinical data of the two groups were collected, and the levels of serum Hcy, VB 12 and D-D/R were detected, to analyze the relationship between levels of serum Hey, VB 12, D-D/R and pregnancy outcome. Results The serum Hey and D-D/R levels of normal pregnancy group were significantly lower than those of threatened abortion group （P 〈0.05）, and the serum VB12 level was significantly high than that of threatened abortion group （P 〈0.05）. Spearman correlation analysis showed that serum Hey and D-D/R levels had positive correlations with the inevitable abortion （r = 0.824, 0.819; P〈0.05）, serum VB12 level had negative correlation with the inevitable abortion （rs = -0.633, P〈0.05）, serum Hey and D-D/R levels had negative correlations with successful protection of fetus （rs = -0.687, -0.614; P 〈0.05）, and serum VB 12 level had positive correlation with successful protection of fetus （r, = 0.642, P 〈0.05）. Conclusions The combined detection of serum Hcy, VB12 and D-D/R has certain clinical si^aificance for evaluating threatened abortion and pregnancy outcome.
