摘要
目的:探讨尿微量白蛋白(mALB)、β_2微球蛋白(β_2-MG)、N-乙酰β_2-D-氨基葡萄糖苷酶(NAG)联合检测对早期糖尿病肾病(DN)的临床意义。方法:采用诊断金标准将236例疑似糖尿病患者确诊分为DN组117例及非DN组119例,所有研究对象检测尿mALB、β_2-MG、NAG。结果:DN组的mALB、β_2-MG和NAG水平分别明显高于非DN组,且差异均有统计学意义(P<0.05)。mALB的灵敏度、特异度均高于β_2-MG、NAG,漏诊率、误诊率均低于β_2-MG、NAG。三项联合检测的灵敏度、特异度均高于三种单项检测,漏诊率、误诊率均低于三种单项检测。结论:采用尿mALB、β_2-MG、NAG联合检测,可及时反映患者肾脏受损情况,有利于糖尿病继发性肾功能损害的早期预防和治疗。
Objective To discuss the clinical significance of combined detection of urinary microalbumin(mALB),β2-microglobulin(β2-MG),N-acetyl-β-D-glucosaminidase(NAG) in early diabetic nephropathy(DN). Method 236 cases of suspected diabetic patients were divided into DN group(117 cases) and non-DN group(119 cases). All subjects were examined for urinary mALB,β2-MG,and NAG. Results The levels of mALB,β2-MG and NAG of DN group were all apparently higher than that of non-DN group. The differences were statistically significant(P〈0. 05). The sensitivity and specificity of mALB were higher than that of β2-MG and NAG. The rate of misdiagnosis and misdiagnosis were lower than that of β2-MG and NAG. The sensitivity and specificity of the three combined tests were higher than those of the three individual tests. The rate of misdiagnosis and misdiagnosis were lower than those of the three individual tests. Conclusion The combined detection of urinary mALB,β2-MG and NAG can reflect the renal damage of the patients in time. It is beneficial to the early prevention and treatment of secondary renal damage in diabetes mellitus
