关节镜下缝线桥技术修复肩袖全层撕裂疗效分析 被引量:5

摘要 目的探讨关节镜下采用缝线桥技术(suture bridge)修复全层肩袖损伤的临床疗效。方法 2011年1月至2014年6月,对69例全层肩袖损伤患者予以关节镜下缝线桥技术修复,并对随访结果进行统计学分析。随访24个月,牛津大学肩关节评分(Oxford Shoulder Score,OSS)、美国加州大学肩关节评分系统(University of California at Angeles,UCLA)和Constant-Murley评分表(Constant-Murley Score,CM)评分系统对术后患侧肩关节功能进行评分。结果术后OSS、UCLA和CM评分较术前均明显提高,差异有统计学意义(P<0.05)。前屈、内旋和外旋角度术后24个月随访结果较术前均明显提高,差异有统计学意义(P<0.05)。但外展角度术后24个月随访结果较术前差异无统计学意义(P>0.05)。VAS和EQ-5D生活质量评分系统(Euro-QoL 5-Dimensions Questionnaire score)术后24个月随访评分较术前均明显提高(P<0.05)。结论肩关节镜下缝线桥技术在修复全层肩袖损伤方面是一种可靠且有效的肩袖修复技术。 Objective To evaluate the clinical efficacy of arthroscopic suture bridge technique for repairing full-thickness rotator cuff tears. Methods Clinical date of 69 patients with full-thickness rotator cuff tear,who were treated with arthroscopic suture bridge technique from January 2011 to June2014,were retrospectively reviewed. Patients were followed-up for 24 months after surgery,and surgical outcomes were evaluated with Oxford Shoulder Score( OSS),University of California at Angeles( UCLA) and Constant-Murley( CM) scales. Results The OSS,UCLA and CMscores 24 months after surgery were all higher than those before surgery( P 0. 05). The means of anteflexion,internal rotation and extorsion angles were better than those before surgery( P〈0. 05); but there was no significant difference in abduction angles between that before and after surgery.( P〈0. 05). The means of VAS and Euro Qo L 5-Dimensions Questionnaire scores after 24 months follow-up were significantly increased( P〈0. 05). Conclusion Arthroscopical suture bridge is a reliable and effective technique for repairing full-thickness rotator cuff tears.
