改良髌旁外侧入路用于外翻膝人工全膝关节置换术的疗效

Clinical effect of improved lateral patellar approach in valgus knee patients with total knee arthroplasty

目的探讨改良髌旁外侧入路用于外翻膝人工全膝关节置换术的疗效。方法选取于我院行人工全膝关节关节置换术的外翻膝患者52例,随机分为对照组与试验组,各26例。对照组按照传统髌旁外侧入路治疗,试验组则采用改良髌旁外侧入路治疗。比较两组患者外翻角度、膝关节活动度及膝关节功能。结果术后,两组患者外翻角度、股胫角及膝关节活动度明显优于术前,且试验组明显优于对照组(P<0.05)。术后,两组HSS评分(对照组疼痛除外)较术前明显提升,且试验组明显优于对照组(P<0.05)。结论改良髌旁外侧入路用于外翻膝人工全膝关节置换术中效果满意,可有效改善患者外翻膝症状,对促进患者膝关节功能恢复具有重要意义,可作为外翻膝手术治疗的良好入路。

Objective To investigate the clinical effect of improved lateral patellar approach in valgus knee patients with total knee arthroplasty. Methods Fifty-two knee valgus patients who treated with total knee arthroplasty in our hospital were selected and randomly divided into control group and experimental group, with 26 cases in each group. The control group was given routine lateral patellar approach in total knee arthroplasty, and the experimental group was treated with improved lateral patellar approach in total knee arthroplasty. The extroversion angle, femorotibial angle and knee joint function of the two groups were compared. Results After operation, the extroversion angle, femorotibial angle and knee extension range of motion of the two groups were better than those before operation, and those of the experimental group were better than the control group（P〈0.05）. After operation, the HSS scores（except pain in the control group） of the two groups were better than those before operation, and those of the experimental group were better than the control group（P〈0.05）. Conclusion Improved lateral patellar approach in total knee arthroplasty has remarkable clinical effect. It can significantly improve the valgus symptoms, and it has great significance to promote the recovery of knee joint function in patients, which can be good approach to the treatment of valgus knee surgery.
