脑脊液乳酸检测在婴幼儿脑膜炎中的诊断意义 被引量:3

摘要 目的探讨脑脊液乳酸检测在婴幼儿脑膜炎中的诊断意义。方法 70例脑膜炎婴幼儿,其中化脓性脑膜炎(化脓性脑膜炎组)33例、病毒性脑膜炎(病毒性脑膜炎组)37例,采用美国GEM Premier 3000全自动血气生化分析仪检测脑脊液乳酸(CSF-LA)水平,并进行组间比较。结果治疗前,化脓性脑膜炎组CSF-LA水平明显高于病毒性脑膜炎组,差异具有统计学意义(P<0.05);治疗1周后化脓性脑膜炎组CSF-LA水平为(1.23±0.31)mmol/L,与治疗前的(2.63±0.17)mmol/L比较差异有统计学意义(P<0.05);而病毒性脑膜炎组患儿经治疗1周后CSF-LA水平为(0.76±0.14)mmol/L,与治疗前的(0.84±0.21)mmol/L比较差异无统计学意义(P>0.05)。结论 CSF-LA应作为婴幼儿脑膜炎的常规检测项目,为临床综合诊断和治疗提供有利的证据。 Objective To investigate the diagnostic significance of cerebrospinal fluid lactic acid detection in infantile meningitis. Methods There were 70 meningitis infants, including 33 cases f purulent meningitis (purulent meningitis group) and 37 cases of viral meningitis (viral meningitis group). The level of cerebrospinal fluid lactic acid (CSF-LA) was detected by GEM Premier 3000 automatic blood biochemistry analyzer and compared between two groups. Results Before treatment, purulent meningitis group had obviously higher CSF-LA level than viral meningitis group, and the difference was statistically significant (P〈0.05). After 1 week of treatment, purulent meningitis group had statistically significant difference in CSF-LA level as (1.23±0.31)mmol/L, comparing with (2.63±0.17)mmol/L before treatment (P〈0.05). Viral meningitis group had no statistically significant difference in CSF-LA level as (0.76±0.14)mmol/L, comparing with (0.84±0.21)mmol/L before treatment (P〉0.05). Conclusion CSF-LA should be used as a routine examination for infantile meningitis, so as to provide favorable evidence for clinical diagnosis and treatment.
