腹腔穿刺置管系统在脑室-腹腔分流术的疗效分析观察 被引量:1

摘要 目的研究在脑室-腹腔分流术中应用腹腔穿刺置管系统的效果。方法 36例脑积水患者,均应用腹腔穿刺置管系统,通过脐周四点提起法将腹壁提起,通过脐缘穿刺置入脑室-腹腔分流管腹腔端,对置管过程进行观察,分析结果。结果 36例患者均成功置管,置管成功率为100%,穿刺置管时间最短3 min,最长5 min,置管中无腹腔内脏器损伤、腹腔内出血出现。通过观察发现穿刺系统未损伤血管或者腹腔脏器,分流管腹腔端可以准确完成盆腔的导向。36例患者在手术结束后接受腹部X线平片复查,结果发现分流管处在腹腔中,无打结情况及反折情况,穿刺点无遗留瘢痕。36例患者均接受随访,随访时间最短5个月,最长7个月,随访期间无腹壁疝、肠粘连、腹腔内脏器损伤等并发症出现,获得满意分流效果。结论腹腔穿刺置管系统在脑室-腹腔分流术中应用微创、操作简便、安全性高、成功率高,不会有瘢痕遗留等。 Objective To study the effect of peritoneal puncture and catheterization system in ventriculo-peritoneal shunt. Methods A total of 36 patients with hydrocephalus all received peritoneal puncture and catheterization system. The abdominal wall was lifted by the umbilical point lifting method and placed through the umbilicus puncture into the abdominal cavity of the ventricle-peritoneal shunt. The procedure was observed and the Results were analyzed. Results All 36 cases were successfully placed, and the success rate of catheterization was 100%, with shortest puncture time as 3 min and longest as 5 min. There was no abdominal visceral injury and intraperitoneal hemorrhage in the catheterization. Through observation, it was found that the puncture system did not damage the blood vessels or abdominal organs, and the distal end of the shunt tube could accurately guide the pelvic cavity. 36 patients received abdominal X-ray examination at the end of the operation, and the Results showed that the shunt tube was in the abdominal cavity. There was no knot situation and reflexion, and no scar in the puncture point. All the 36 patients were followed up for a minimum of 5 months and a maximum of 7 months. No complications such as abdominal wall hernia, intestinal adhesion, and intra-abdominal organ injury occurred during the follow-up period. Satisfactory shunting Results were obtained. Conclusion Peritoneal puncture and catheterization system is minimally invasive, simple and safe, with high success rate and no scar left behind in ventriculo-peritoneal shunt.
