降钙素原、脑钠钛前体与儿童重症化手足口病的相关性研究 被引量:1

摘要 目的:探讨降钙素原、脑钠钛前体在手足口病(HFMD)重症化患儿诊断中的临床应用价值。方法:选取84例重症HFMD患儿为重症组,另选同期96例单纯病毒感染HFMD患儿为单纯病毒组、86例非重症HFMD患儿合并细菌感染为非重症合并细菌组。所有患儿入院24 h内均予降钙素原(PCT)、C-反应蛋白(CRP)、白细胞(WBC)及肌钙蛋白I(c Tn I)、肌酸激酶同工酶(CK-MB)、脑钠钛前体(NT-pro BNP)检测。采用单因素方差分析,差异有统计学意义(P<0.05)。结果:HFMD重症组PCT、NT-pro BNP水平较非重症合并细菌组、单纯病毒组均高,差异有统计学意义(P<0.05)。非重症合并细菌组PCT、NT-pro BNP水平较单纯病毒组高,差异有统计学意义(P<0.05)。而三组中WBC、CRP、CK-MB及c Tn I水平比较,差异均无统计学意义(P>0.05)。结论:快速检测PCT、NT-pro BNP水平可客观评估HFMD患儿病情,有助于早期识别重症化HFMD患儿,即重症化HFMD与PCT、NT-pro BNP水平密切相关。 Objective To evaluate application values of procalcitonin (PCT) and N-terminal pro-brain natriuretic peptide(NT-proBNP) in early diagnosis of patients with severe hand, foot and mouth disease (HFMD). Methods: 84 children with severeHFMD were set as severe HFMD group, 96 cases with HFMD and simple viral infection were used as simple viral infection group, and96 cases of non severe HFMD combined with bacterial infections were selected as non-severe combined with bacterial infection group.The levels of PCT, C-reactive protein (CRP), while blood cell (WBC), troponin I (cTnI), creatine kinase-MB (CK-MB) and NT-proBNP of these children within 24 hours of admission were detected. One-way AVONA was used, P〈0. 05 as statistically significant. Results: The levels of PCT and NT-proBNP in the severe HFMD group were higher than those of the simple viral infection groupand non-severe combined with bacterial infection group, and the differences were statistically significant (P〈0. 05). The levels of PCTand NT-proBNP in the non-severe combined with bacterial infection group were higher than those of the simple viral infection group,and the differences were statistically significant (P〈0. 05). There were no statistically significant differences in the levels of WBC,CRP, CK-MB and cTnI among the three groups (P〉0. 05). Conclusions: The rapid detection of the levels of PCT and NT-proBNPcan evaluate the severity of HFMD in the children and facilitate the early diagnosis of severe HFMD, as they have close correlationswith severe HFMD.
