多点式导管接触性溶栓联合系统性溶栓治疗股青肿疗效分析

Analysis on effect of multi-point catheter-directed thrombolysis and systemic thrombolysis combination in treatment of phlegmasia cerulea dolens

目的:观察导管接触性溶栓联合系统性溶栓治疗股青肿的临床效果。方法:将57例股青肿患者随机分为置管溶栓组和手术切开取栓对照组,比较两组患者术前及溶栓后1、7、14 d健、患侧肢体膝下10 cm周径差,测定术前及溶栓后1、7、14 d健、患侧足皮温,并进行疗效评价。结果:术前及术后1、7、14 d,两组患者健、患侧肢体周径差、患足皮温均无统计学意义(P>0.05)。结论:导管接触性溶栓联合系统性溶栓治疗股青肿,具有创伤小、风险小、术中出血少,能有效避免取栓术中对瓣膜的破坏,降低术后下肢静脉瓣膜功能不全并发症的发生率。

Objective： Observe the clinical effect of multi-oint catheter-directed thrombolysis（ CDT） and systemic thrombolysis combination in treatment of phlegmasia cerulea dolens（ PCD）. Methods： 57 patients with phlegmasia cerulea dolens were randomly divided into a catheter-irected intrathrombus thrombolysis group and an operative thrombectomy control group,the perimeter difference between parts 10 cm below the knees of the suffered limb and the healthy one of the two groups were compared before and on the 1 st,7 th and 14 th day after thrombolysis,and the skin temperature of the suffered foot and the healthy one was measured before and on the 1 st,7 th and 14 th day after thrombolysis,and the treatment effect was evaluated. Results： The perimeter difference between of the suffered limb and the healthy one and the skin temperature of the suffered foot and the healthy one of the two groups had no statistical significance（ P〈0. 05）. Conclusion： The multi-point catheter-directed thrombolysis and systemic thrombolysis combination in treatment of phlegmasia cerulea dolens is small in wound,low in risk,less in hemorrhage during operation,capable of effectively avoiding the damage of embolectomy to valve,as well as decreasing the incidence of postoperative complication of venous valvular incompetence of lower extremity.
