老年高血压肾损害患者凝血指标的变化 被引量:6

The Changes of Blood Coagulation Indexes in Elderly Patients with Hypertension Renal Damage

摘要 目的探讨高血压肾损害患者凝血指标的变化,指导临床用药。方法对22例高血压老年患者(高血压组)和56例高血压肾损害老年患者(高血压肾损害组)进行aPTT、PT、TT、Fbg、D二聚体(D-D)及血肌酐(Cr)等指标的检测,比较分析两组患者各项指标的变化,同时分析高血压肾损害组Fbg与估算肾小球滤过率(eGFR)的关系。结果高血压肾损害组与高血压组患者的aPTT、PT、TT比较均无明显差异(P>0.05);Fbg及D-D明显升高(P<0.05)。eGFR与Fbg呈负相关,随肾功能的恶化,Fbg表达增加。结论老年高血压肾损害患者处于明显的高凝状态。Fbg与高血压患者肾损害密切相关,可作为评估高血压肾损害患者凝血与纤溶状态的指标,指导临床用药。 Objective To observe the changes of blood coagulation indexes in elderly patients with hypertension renal damage and to guide the clinical medication. Methods Elderly patients were divided into two groups, with 22 in hypertension group (hypertension group)and 56 in hypertension renal damage group (hypertension renal damage group). Data on aPTT, PT, TT, Fbg and D-dimer (D-D)were collected for all patients, and the two groups were compared for the differences in the changes datum. In addition, data from hypertension renal damage group were analyzed for any potential association of eGFR with Fbg. Results There were no differences in aPTT, PT, TT between hypertension renal damage group and hypertension group. Compared with hypertension group, hypertension renal hypertension renal damage damage group showed higher Fbg and D-D (P 〈 0. 05 ) group, eGFR levels were negatively correlated with Fbg. With the deterioration of renal function, the Fbg increased. Conclusion Elderly patients with hypertension renal damage showed an hypereoagulable state, obviously. Fbg were closely related to renal damage in patients with hypertension. They can be used as an indicator to evaluate the coagulation and fibrinolysis in elderly patients with hypertension renal damage, and instructed drug therapy.
