初探糖肝煎浓缩丸对2型糖尿病合并非酒精性脂肪肝大鼠的治疗作用

Preliminary investigation on therapeutic effects of Tangganjian Concentrated Pills in rats with type 2 diabetes mellitus and nonalcoholic fatty liver

目的:初步研究糖肝煎浓缩丸对2型糖尿病合并非酒精性脂肪肝模型大鼠糖脂代谢,肝功能及肝脏胰岛素受体的影响。方法:选择健康雄性Wistar大鼠进行研究,随机分为正常组,模型组,吡格列酮组,糖肝煎浓缩丸中剂量组和糖肝煎浓缩丸低剂量组。采用高脂高糖饲料喂养加30 mg·kg-1链脲佐菌素(STZ)腹腔注射的方法建立2型糖尿病合并非酒精性脂肪肝的动物模型。吡格列酮组给予吡格列酮混悬液1 mg·kg-1,糖肝煎浓缩丸中剂量组给以5.6 g·kg-1·d-1糖肝煎浓缩丸混悬液,糖肝煎浓缩丸低剂量组以2.8 g·kg-1·d-1糖肝煎浓缩丸混悬液灌胃,模型组和正常组给予相同量的生理盐水。8周后,检测血糖,血脂,肝脏功能及胰岛素受体的表达水平。结果:与正常组比较,模型组大鼠空腹血糖(FBG)、餐后2 h血糖(P2h BG)、空腹胰岛素(Fins)升高,TC、TG、LDL-C、AST、ALT均升高,HDL-C和肝糖原水平,胰岛素受体蛋白表达水平均下降(P<0.01);与模型组比较,各治疗组胰岛素抵抗指数改善,TC、TG、LDL-C、ALT、AST均下降,HDL-C,肝糖原,胰岛素受体蛋白表达水平均升高(P<0.01);与吡格列酮组比,糖肝煎浓缩丸组TC、LDL-C降低显著(P<0.01),其余指标差异无显著性;糖肝煎浓缩丸中剂量组和低剂量组相比,糖肝煎浓缩丸中剂量组可以显著降低模型大鼠血糖及血脂水平,改善胰岛素抵抗,肝脏功能及肝脏形态。结论:糖肝煎浓缩丸对2型糖尿病及非酒精性脂肪肝大鼠的糖脂代谢具有显著的治疗作用,明显改善肝脏功能,且增加胰岛素受体蛋白表达水平,改善胰岛素抵抗。

OBJECTIVE To study the effect of Tangganjian（ TGJ） Concentrated Pills on glucose and lipid metabolism and liver function and expression level of insulin receptor protein in type 2 diabetic rats with nonalcoholic fatty liver disease. METHODS The healthy male Wistar rats were selected and randomly divided into the normal group,the model group,the pioglitazone group,the low dose and middle dose groups of TGJ concentrated pills. The type 2 diabetes mellitus combined with nonalcoholic fatty liver disease model rat was established by high fat and high sugar diet and 30 mg·kg^-1 STZ intraperitoneal injection. Rats in the pioglitazone group were treated with 1 mg·m L^-1 pioglitazone suspension,rats in the middle dose group were treated with 5. 6 g·kg^-1·d^-1 TGJ concentrated pill suspension and in the low dose group with 2. 8 g·kg^-1·d^-1 TGJ concentrated pills. Animals in the model group and normal group were given the same volume of normal saline. After 8 weeks of intervention,blood glucose,blood lipid and liver function and expression level of insulin receptor protein were detected. RESULTS Compared with the normal group,the fasting blood glucose（ FBG）,2 h postprandial blood glucose（ P2 h BG）,fasting insulin（ Fins） increased,TG,TC,LDL-C,AST and ALT were increased,HDL-C and liver glycogen levels,insulin receptor protein expression levels were decreased in rats in the model group（ P〈0. 01）.Compared with the model group,insulin resistance index was improved,TC,TG,LDL-C,ALT,and AST were decreased,HDL-C,liver glycogen content and expression level of insulin receptor protein were increased in each treatment group（ P〈0. 01）. Compared with the pioglitazone group,the TC,LDL-C of TGJ concentrated pill group decreased significantly（ P〈0. 01）,the other indexes had no significant difference. Compared with two TGJ concentrated pill groups,the middle dose TGJ concentrated pill group showed significantly reduce the blood glucose and blood lipid level,improved insulin resistance,liver function and liver morphology. CONCLUSION TGJ concentrated pill has significant therapeutic effects on glucose,lipid metabolism and expression level of insulin receptor protein in rats with type 2 diabetes mellitus and nonalcoholic fatty liver,the higher dose is better.
