复方血栓通胶囊治疗糖尿病视网膜病变的成本效果分析

Cost-effectiveness analysis of Compound Xueshuantong Capsule in the treatment of diabetic retinopathy

目的:评价复方血栓通胶囊治疗糖尿病视网膜病变(DR)的经济性。方法:检索中国知网数据库,筛选符合复方血栓通胶囊联合羟苯磺酸钙治疗DR的疗效与单独使用羟苯磺酸钙治疗DR的疗效对比的文献,及符合复方血栓通胶囊联合激光治疗DR的疗效与单独使用激光治疗DR的疗效对比的文献,提取治疗效果数据,成本数据来源于真实临床花费数据,构建决策树模型,从付费者的角度出发,进行成本-效果分析。结果:复方血栓通胶囊联合激光治疗DR有效率为89.78%,总费用为7 153.10元,单独使用激光治疗DR有效率为75.56%,总费用为8 096.99元。复方血栓通胶囊联合羟苯磺酸钙治疗DR有效率为87.37%,总费用为7 126.86元,单独使用羟苯磺酸钙治疗DR有效率为65.27%,总费用为10 511.24元。复方血栓通胶囊联合羟苯磺酸钙或激光治疗DR的有效率更高,成本更低。结论:复方血栓通胶囊联合羟苯磺酸钙或者联合激光治疗DR与单独使用羟苯磺酸钙或者激光治疗DR具有明显成本效果优势,值得推广应用。

OBJECTIVE To evaluate the clinical and economic benefits of Compound Xueshuantong Capsule in the treatment of diabetic retinopathy（ DR）. METHODS China Knowledge Network Database was searched to identify the studies illustrating the clinical efficacy of combination treatment of Compound Xueshuantong Capsule with calcium dobesilate compared with calcium dobesilate alone in the treatment of DR and also the studies to illustrate the clinical efficacy of combination treatment of Compound Xueshuantong Capsule with laser treatment compared with laser treatment alone in the treatment of DR. Based on the efficacy data,economic model was constructed to assess the cost effectiveness of the compound Xueshuantong combination therapy compared withsingle therapy. Cost data were abstracted to populate the model from real clinical cost data,including thirdparty electronic drug trading platform,expert advice,literature review and KOL surveys. RESULTS Compound Xueshuantong Capsule in combination with laser in the treatment of DR achieved a total response rate of 89. 78 % vs. 75. 56 % for the laser alone treatment,with a total cost of 7 126. 86 yuan and10 511. 24 yuan,respectively. Compound Xueshuantong Capsule in combination with calcium dobesilate in the treatment of DR achieved a total response rate of 87. 37 % vs. 65. 27 % for the calcium dobesilate alone,with a total cost of 7 126. 86 yuan and10 511. 24 yuan,respectively. CONCLUSION Compound Xueshuantong Capsule in combination with calcium dobesilate or laser achieved better clinical efficacy and overall lower costs when compared with calcium dobesilateor laser alone.
