等级反应模型在高血压病中医证候研究中的应用

The Application of Graded Response Model to Syndrome Differentiation

目的以高血压病的肝阳上亢证为例,探讨其四诊信息的效度、等级划分的合理性及重要性。方法利用R语言ltm包中的等级效应模型对1280例高血压病的肝阳上亢证进行分析,以得到肝阳上亢证的区分度系数与难度系数的估计值。结果所有指标的区分度参数a均大于0.4,说明这12个条目的效度均较好,能较好反映肝阳上亢证的结构。结论应用等级反应模型研究证候规范标准是可行的。

Objective Taking hyperactivity of liver yang caused by hypertension as an example,to explore the validity of the rationality and importance of classification by grades. Methods Graded response model was used in the ltm package of R language to analyze 1280 cases of hypertension of liver yang, in order to get estimated values of discrimination validity and diffi- culty coefficient for hyperactivity of liver yang caused by hypertension. Results The results showed that discrimination validity parameters of all indexes were over 0. 4 ,which illustrates the validity of 12 items are good and they can better reflect the struc- ture of hyperactivity of liver yang. Conclusion Application of graded response model to research syndrome standard level is feasible.
