山东省某三甲医院2006年与2016年住院患者疾病特征对比分析

Comparison of the characteristics of inpatients' disease distribution in a tertiary hospital in Shandong Province in 2006 and 2016

目的比较分析山东省某三甲医院2006年和2016年住院患者疾病分布变化趋势。方法根据ICD-10分类标准,对山东省某三甲医院2006年与2016年的住院患者信息进行整理,运用描述性统计和χ2检验分析各系统疾病构成比变化,采用相关系数分析住院人数变化趋势,应用两独立样本t检验分析平均住院日的变化。结果该院2016年住院人数是2006年的3.53倍,月住院人数变化趋势总体一致(r=0.930);男女性别比由2006年1.05∶1下降到2016年0.92∶1,60岁及以上住院患者增长近8个百分点;前10位各系统疾病住院人数构成比顺位发生明显变化(P<0.001),消化系统、泌尿生殖系统、神经系统和妊娠分娩产褥疾病患者构成比顺位上升,呼吸系统下降;与2006年相比,2016年肺癌、甲状腺恶性肿瘤、癫痫占比靠前,且上升幅度较大。结论该院慢性病住院人数增长较快,各系统疾病构成变化明显,应根据疾病分布特征变化,合理调配卫生资源。

Objective To compare the characteristics of inpatients＇ disease distribution in a tertiary hospital in. Shan- dong Province in 2006 and 2016. Methods The related data were classified according to ICD-10 based on the discharge diagnosis. The changes of disease constitution ratio, number of inpatients, and length of hospital stay were analyzed with descriptive statistics andx2 test, correlation coefficient, and t-test, respectively. Results The number of inpatients in 2016 was 3.53 times of that in 2006. The changing trends of monthly number of inpatients in 2006 and 2016 were consistent （r=0.930）. The male to female ratio decreased from 1.05：1 in 2006 to 0.92：1 in 2016. The number of inpa- tients aged 60 or above increased by 8%. The constitution ratio of the top 10 systemic diseases changed significantly in 2006 and 2016 （P〈0.001）. Diseases of the digestive, genitourinary and nervous systems and puerperal diseases in- creased in rank, while diseases of the respiratory system decreased in rank. In 2016, lung cancer, thyroid carcinoma and epilepsy accounted for a large proportion and increased sharply. Conclusion The number of inpatients with chronic dis- eases increased rapidly and the constitution ratio of diseases changed obviously. Health resources should be allocated rea- sonably based on the new features of disease distribution.
