腹膜透析患者体力活动状况及其影响因素分析 被引量:2

Physical activity and influencing factors in peritoneal dialysis patients

摘要 目的分析长期腹膜透析患者的体力活动状况,探讨影响腹膜透析患者体力活动状况的因素。方法回顾性横断面研究。将146例腹膜透析患者分为体力良好组和体力不良组,比较2组间相关指标,分析影响患者体力状况的因素。结果纳入的146例患者中,平均年龄(51.14±14.15)岁,男性86例(58.9%)。根据活动指数分为体力良好组(n=66)和体力不良组(n=80),体力良好组在营养、躯体健康、心理健康、生活质量比体力不良组好,体力不良组焦虑、抑郁发生率比体力良好组高,体力良好组的平均年龄较低(P<0.05)。单因素相关分析得出躯体健康、生活质量与活动呈正相关,年龄、焦虑、抑郁、合并症与活动呈负相关。多元线性回归分析发现年龄(P<0.05)、躯体健康(P<0.05)和合并症(P<0.05)是体力的独立危险因素。结论年龄、焦虑、抑郁、生活质量、躯体健康及合并症是影响患者体力状况的因素,其中年龄、躯体健康及合并症是独立的危险因素。 Objective To analyze the physical activity status of maintenance peritoneal dialysis patients, and to explore the influencing factors of Karnofsky activity index. Methods A retrospective cross - sectional study was conducted. A total of 146 patients treated on continuous ambulatory peritoneal dialysis for at least 3 months were enrolled, patients were divided into physical well ( n = 66) and bad (n = 80) groups according to Karnofsky performance status. The clinical indicators were observed and compared. Through single factor correlation analysis and multiple linear regression analysis to find factors affected the physical activity status in peritoneal dialysis patients. Results In 146 patients, average age was ( 51. 14 ± 14. 15 ) years, male 86 cases ( 58. 9% ), female 60 cases (41.1% ). Compared with bad physical group, the nutritional status, Physical Component Summary (PCS), Mental Component Summary ( MCS), SF - 36 score were significantly better in good physical group ( P 〈 0. 05 ). Good physical group had lower anxiety and depression score, the average age of physical good group was lower (P 〈 0. 05). There were no significant differences in the nationality, age, gender, causes of disease, PD duration, Body Mass Index (BMI) and Charlson comorbidity index between good and bad physical group (P 〉 0.05 ). Age, anxiety, depression, quality of life, physical health and Charlson comorbidity index were the main risk factors of physical condition used single factor correlation analysis and multiple linear regression. And physical health and SF -36 were positively correlated with KFS scores. Age, anxiety, depression and Charlson comorbidity index were negatively correlated with KFS score. Age ( P 〈 0. 05 ) , physical health ( P 〈 0. 05 ) and comorbidity ( P 〈 0.05 ) were independent risk factors of KFS. Conclusion Age, anxiety, depression, quality of life and physical health are the influence factors of physical activity status, age and physical health are independent risk factors.
