原发性高血压早期肾损害与血清25-(OH)D水平的相关性 被引量:2

Correlation between early renal damage and serum 25(OH)D levels in EH patients

摘要 目的探讨原发性高血压(EH)患者早期肾损害与血清25-羟维生素D[25-(OH)D]水平的相关性。方法收集EH患者361例,将患者分为EH组和EH+尿微量白蛋白(m ALB)组。查询患者临床一般资料,并检测实验室指标,包括常规肾功、血脂指标以及25-(OH)D、m ALB。分析m ALB与25-(OH)D水平的相关性。结果 2组患者的病程、BMI、TC、LDL-C、入院SBP、入院DBP、脉压差异有统计学意义(P<0.05)。EH+m ALB组的血清25-(OH)D水平低于EH组(P<0.05)。2组中25-(OH)D缺乏和不足者共357例,占98.9%。m ALB水平与病程、BMI、TG、入院SBP、入院DBP呈正相关,与25-(OH)D呈负相关(P<0.05)。校正性别、年龄、病史等因素后,血清25-(OH)D水平与入院SBP、m ALB水平呈负相关,与HDL-C水平呈正相关(P<0.05)。结论 EH患者存在25-(OH)D缺乏或不足,这与早期肾损害的发生、发展密切相关。 Objective To investigate the correlation between early renal injury and serum 25 (OH) D in EH patients. Methods Atotal of 361 patients with EH were enrolled in the cardiovascular disease hospital of Ningxia Medical University from March to December in 2016. The patients were divided into EH group and EH + mALB group according to the 2012 KDIGO Guide. The patient general information was calculated, laboratory test index were detected, including routine renal function indexes, blood lipid Indexes, 25 (OH) D and mALB. The correlation between MALB and 25 - (OH) D was analyzed. Results The course of disease, BMI, TC, LDL - C, SBP, DBP and pulse pressure of two groups were statistically significant ( P 〈 0.05 ). The level of 25 - (OH) D in EH + mALB group was lower than that in EH group (P 〈 0.05 ). The patients of 25 - (OH) D deficiency were 357 cases in two groups, accounting for 98.9%. The level of mALB was positively correlated with the course of disease, BMI, TG, SBP, DBP and negative correlation with 25 - (OH) D ( P 〈 O. 05 ). After adjusting the gender, age, history of correction factors, the level of serum 25 - (OH) D was negatively correlated with SBP and mALB, with being positively correlated with HDL- C (P 〈 0.05 ). Conclusion 25 - (Oil)D deficiency is closely related to the occurrence and development of early renal damage.
