大株红景天注射液联合尼可地尔治疗冠心病的疗效观察

The effect observation of nicorandil in the treatment of coronary heart disease combined with Sofren injection

目的观察大株红景天注射液联合尼可地尔对冠心病的治疗效果。方法选取2014年2月至2017年9月接受治疗的264例患者为研究对象进行前瞻性研究,按照随机数表将患者平均分为两组,每组132例。对照组患者采用尼可地尔进行治疗,观察组患者采用大株红景天注射液联合尼可地尔治疗。采用血液分析仪对患者进行血液相关检查(低切变率下全血黏度、高切变率下全血黏度、血浆D-二聚体、纤维蛋白原等指标),并记录患者心电图变化(心电图ST段压低的发生次数和累计持续时间及心肌缺血总负荷),采用改良冠心病疗效评价量表对患者进行疗效评价。结果观察组患者低切变率下全血黏度[(6.35±0.38)m Pa·s]、高切变率下全血黏度[(4.98±0.67)m Pa·s]、血浆D-二聚体含量[(562.15±168.22)g/L]、纤维蛋白原含量[(4.08±0.51)μg/L]低于对照组患者(P<0.05)。观察组患者ST段压低的发生次数[(3.14±1.26)次/24 h]、ST段压低累计持续时间[(24.16±9.32)min]、肌缺血总负荷[(41.69±16.28)mm·min]均显著低于对照组患者(P<0.05)。观察组患者总有效率(96.97%)显著高于对照组(81.06%),差异有显著性(P<0.05)。结论大株红景天注射液联合尼可地尔可以显著降低冠心病患者的血浆D-二聚体、纤维蛋白原含量等血流动力学指标水平,改善心肌供血,疗效显著。

Objective To explore therapeutic effect of nicorandil on coronary heart disease combined with Sofren injection. Methods264 patients from February 2014 to September 2017 in our hospital were selected as the research object. According to the random number table,the patients were divided into two groups,132 cases in each group. Control group were treated with nicorandil treatment,and the observation group were treated by combined treatment of Sofren injection nicorandil. The blood analyzer was used for some blood examinations of patients,ECG ST segment changes were recorded,coronary heart disease assessment scale was performed to evaluate the curative effect of the patients. Results The observation group of patients with low shear rate of whole blood viscosity was（ 6. 35 ± 0. 38） m Pa·s,high shear rate of whole blood viscosity was（ 4. 98 ± 0. 67） m Pa·s,plasma D-D dimer content was（ 562. 15 ± 168. 22） g/L,fibrinogen was（ 4. 08 ± 0. 51） g/L,which was lower than that of the control group（ P〈0. 05）. ST segment depression incidence in observation group of patients was（ 3. 14 ± 1. 26）/24 h,ST segment depression cumulative duration was（ 24. 16 ± 9. 32） total load min,myocardial ischemia was（ 41. 69 ± 16. 28） mm· min,which were significantly lower than those of the control group（ P〈0. 05）. The total effective rate of the observation group was 96. 97%,which was significantly higher than that of the control group,81. 06%（ P〈0. 05）. Conclusion Sofren injection combined with nicorandil can significantly reduce the plasma D-D dimer,fibrinogen and other hemodynamic parameters,improve myocardial blood supply,has remarkable curative effect.
