临床病例讨论——以乏力、水肿为首发表现的原发性甲状旁腺功能亢进症

Hypodynamia and edema as the first manifestation of primary hyperparathyroidism

目的分析原发性甲状旁腺功能亢进症的临床特点,提高临床医生对甲状旁腺腺瘤的认识。方法分析赤峰市医院内分泌科诊断的1例甲状旁腺功能亢进症患者的临床特点,并对相关文献进行总结。结果以乏力、水肿首发,实验室检查存在肾功能不全,除外泌尿系结石及原发性肾脏病变导致;存在贫血,除外肾脏病变及血液疾病导致,经过详细诊治考虑为原发性甲状腺功能亢进、甲状旁腺腺癌所致。结论原发性甲状旁腺功能亢进症临床表现多样,误诊、漏诊率高,需提高临床认识。

Objective To investigate the clinical feature of primary hyperparathyroidism. Methods The diagnosis and treatment of a patient with primary hyperparathyroidism in Chifeng Municipal Hospital were retrospectively analyzed and the related literatures were reviewed. Results Hypodynamia and edema was the first manifestation. The laboratory examination showed a renal insufficiency, with the exception of urinary calculi and primary kidney disease, and anemia, with the exception of kidney disease and blood disease. After a detailed treatment, primary thyroid function hyperfunction and parathyroid gland carcinoma were confirmed. Conclusion There is a variety of primary hyperparathyroidism in clinical manifestation, which has a higher rate of misdiagnosis, so it is necessary to improve clinical knowledge.
