排尿行为健康教育对膀胱过度活动症患者结局变化趋势的预测作用研究

Predictive effect of urination behavior health education on outcome of patients with overactive bladder

目的探讨健康信念模式的排尿行为健康教育对膀胱过度活动症患者的膀胱症状和症状困扰变化趋势的预测作用。方法将104例膀胱过度活动症患者随机分为观察组51例和对照组53例。对照组发放OAB基本知识手册,观察组在发放手册基础上进行排尿行为健康教育。采用增长混合模型分析两组患者6个月内的尿急严重性和症状困扰的轨迹以反映其变化趋势,Logistic分析干预措施对变化趋势的预测作用。结果 104例患者6个月尿急严重性变化趋势有两类:维持状态和改善趋势;症状困扰变化趋势有3类:基线高困扰后维持、基线中等困扰后维持、基线低困扰后改善。干预措施可以显著预测患者的尿急严重性和症状困扰的改善趋势(P<0.05)。结论基于健康信念模式的排尿行为健康教育可有效改善患者的膀胱症状和症状困扰程度,效果至少可维持6个月,具有临床应用价值。

Objective To study the prediction of toileting behavior health education based on health belief model to the bladder symptom and symptom bothersome in overactive bladder patients.Methods 104 patients were randomly divided into experimental group（n=51）and control group（n=53）.The patients in control group received handbook about basic knowledge of overactive bladder,while the patients in experimental group were treated with toileting behaviors health education combined with the handbook.Growth mixed model was used to analysis the urgency severity and symptom bother trajectories of the 104 patients to reflect the trends,and analyzing the prediction of the intervention to outcome trends.Results During 6 months the trends of 104 patients,urgency severity including sustaining status and improving status,and the symptom bother trends including baseline high bothersome and then sustaining status,baseline moderate bothersome and then sustaining status,baseline low bothersome and then improving status.The intervention can significantly predict improving trends of both above（P〈0.05）.Conclusion The toileting behaviors health education based on health belief model can effectively improve the bladder symptoms and symptom bother of overactive bladder patients.It can sustain 6 months.
