摘要 基于国内外对老年抑郁症的研究成果,文章主要对老年抑郁症的精神影像学客观标记物及其临床应用的问题进行了研究。首先,本文综述了研究的背景、目的和意义,进而对生物标记物的概念内涵进行了界定。其次,系统阐述了老年抑郁症的SMRI、FMRI、DTI的脑区脑网络变化,并在此基础上结合精神影像的客观标记物临床实际应用。通过本文的研究,得到以下结论:老年抑郁症患者在额叶-纹状体-边缘网络中的灰质体积明显降低,支持默认网络失活或者过度激活都与老年抑郁症网络机制相关,且与临床表现以及治疗预后呈一定关系。考虑将上述环路以及脑网络功能性连接变化作为老年抑郁症生物标记物。 Based on the research results of geriatric depression at home and abroad, this paper mainly studies the problems of the objective marker and its clinical application of the psychiatric imaging of geriatric depression. Firstly, this paper summarizes the background, purpose and significance of the research, and then defnes the concept of biomarkers. Secondly, we systematically describe the changes of brain area and network in SMRI, FMRI and DTI of geriatric depression, finally, we summarize the brain changes based on the clinical application of objective marker of psychiatric imaging. We get the following conclusion Patients of geriatric depression had lower grey matter volumes in the fronto-striatal-limbic networks. We supported the default network inactivation or over activation, which were all related to the network mechanism of geriatric depression, and were related to clinical manifestations and prognosis. We should consider the above loop and functional connectivity changes of brain network as biomarkers.
