甲状腺功能减退与绝经前女性冠脉病变的相关性研究

摘要 目的探讨甲状腺功能减退与绝经前女性冠脉病变程度的相关性。方法连续入选疑诊冠心病拟行冠脉造影(CAG)的绝经前女性521例,按造影前甲状腺功能分为甲状腺功能减退组(43例)和甲状腺功能正常组(478例)。择期行CAG,运用Gensini评分评价冠脉病变严重程度,并分析TIMI血流情况。结果与甲状腺功能正常组比较,甲减组冠心病发病率显著增高(76.7%比55.9%,P〈0.01),冠脉病变程度更严重(重度:35.9%比18.8%,P〈0.01),慢血流的发生率显著增高(18.6%比2.7%,P〈0.01)。结论甲状腺功能减退症可能是绝经前女性冠心病的危险因素,该类患者冠心病检出率高、冠脉病变严重,且易合并冠脉血流速度慢。 Objective To investigate the correlation between hypothyroidism and the severity of coronary artery lesions in premenopausal women. Methods 521 consecutive suspected premenopausal female patients with coronary artery disease(CAD) who undergone coronary angiography (CAG) were selected. The patients were divid- ed into two groups based on thyroid function prior to CAG: hypothyroidism group (43 patients ) and enthyroidism group (478 patients ). All the patients underwent elective CAG, then the severity of coronary artery lesions was as- sessed by the Gensini scores, and the result of thrombolysis in myocardial infarction (TIMI) was also analyzed. Results Compared with euthyroidism group, the incidence rate of CAD in hypothyroidism group was significantly higher than that in euthyroidism group (76.7% vs. 55.9%, P〈0.01), the severity of coronary artery lesions was greater (proportion of severe patients:35.9% vs. 18.8%, P〈0.01), the incidence rate of slow blood flow was sig-nificantly higher( 18.6% vs. 2.7%, P〈0.01 ). Conclusion Hypothyroidism may be an risk factor in premenopausal women with CAD, which has a higher incidence rate of CAD, an increase in the severity of coronary artery le- sions, and was prone to have slow blood flow.
