尿巨噬细胞趋化蛋白-1和巨噬细胞集落刺激因子对狼疮肾炎复发的诊断作用

摘要 目的探析在狼疮肾炎复发诊断中尿巨噬细胞趋化蛋白-1和巨噬细胞集落刺激因子的临床作用。方法择取2014年1月至2015年1月我院收治的72例狼疮肾炎患者,所选患者均经过环磷酰胺、泼尼松诱导治疗成功,对所选患者的实验室资料及临床资料进行记录,在治疗6个月时对所选患者的尿巨噬细胞趋化蛋白-1和巨噬细胞集落刺激因子进行检测,追踪疾病复发情况。结果经过27个月的随访,20例患者出现一次及以上复发现象,复发时间为(14.6±4.5)个月。与未复发组患者相比,复发组患者尿巨噬细胞趋化蛋白-1和巨噬细胞集落刺激因子均比较高(P<0.05);随访发现,复发组患者血3C减低患者占比、抗ds DNA抗体阳性患者占比均低于非复发组(P<0.05)。结论在狼疮肾炎复发诊断中尿巨噬细胞趋化蛋白-1和巨噬细胞集落刺激因子可以作为重要指导,对狼疮肾炎患者治疗有一定指导作用。 Objective To explore clinical effect of urinary macrophage chemoattractant protein-1 and macrophage colonystimulating factor on diagnosis of lupus nephritis recurrence. Methods Choose 72 cases lupus nephritis patients treated in our hospital from January 2014 to January 2015, who were cured successfully with cyclophosphamide and prednisone inducement. Record laboratory and clinical data of patients, detect urinary macrophage chemoattractant protein-1 and macrophage colony-stimulating factor of patients 6 months after treatment, to track recurrence of disease. Results After 27 months of follow-up, 20 patients recurred one time or more, with recurrence time of(14.6±4.5) months. Compared with non recurrent group, urine macrophage chemokine-1 and macrophage colony stimulating factor of recurrent group was higher,(P〈0.05), follow-up group showed 3 C reduction ratio, anti ds DNA antibody positive ratio of recurrent group was lower than non recurrent group,(P〈0.05). Conclusion In diagnosis of lupus nephritis recurrence, urinary macrophage chemoattractant protein-1 and macrophage colony stimulating factor can be an important guide, which has some certain guiding effect in treatment of lupus nephritis.
