阿托伐他汀干预急性脑梗死的疗效及对MMP-9水平、CVR及其评价指标的影响 被引量:1

摘要 目的:分析阿托伐他汀干预急性脑梗死的疗效及对基质金属蛋白酶-9(MMP-9)水平、脑血管反应性(CVR)及其评价指标的影响。方法:收治急性脑梗死患者127例,根据治疗方法分为两组。对照组给予常规治疗,观察组在常规治疗基础上给予阿托伐他汀治疗,比较两组临床疗效及治疗前后炎性因子水平、MMP-9水平、CVR、MBI评分及颈动脉斑块变化。结果:观察组临床总有效率90.63%,高于对照组,差异有统计学意义(P<0.05)。观察组治疗后hs-CRP、IL-6及MMP-9水平低于对照组,差异有统计学意义(P<0.05)。观察组治疗后CVR、BHI水平及MBI评分高于对照组,差异有统计学意义(P<0.05)。观察组治疗后颈动脉斑块大小、斑块厚度及内-中膜厚度低于对照组,差异有统计学意义(P<0.05)。结论:阿托伐他汀治疗急性脑梗死疗效确切,能降低炎症因子和MMP-9水平,改善脑血管反应性和颈动脉斑块程度。 Objective:To analyze the effect of atorvastatin on acute cerebral infarction and its effect on matrix metalloproteinase-9(MMP-9) level,cerebrovascular reactivity(CVR) and its evaluation index.Methods:127 cases of patients with acute cerebral infarction were divided into two groups according to the treatment method.The control group was given conventional treatment,while the observation group was treated with atorvastatin on the basis of the conventional treatment.The clinical efficacy and the levels of inflammatory factors,MMP-9 level,CVR,MBI score and carotid plaque were compared between the two groups before and after treatment.Results:The total clinical effective rate of the observation group was 90.63%,which was higher than that of the control group,the difference was statistically significant(P〈0.05).The levels of hs-CRP,IL-6 and MMP-9 in the observation group were lower than those in the control group,and the difference was statistically significant(P〈0.05).The scores of CVR,BHI and MBI in the observation group were higher than those in the control group,and the difference was statistically significant(P〈0.05).The carotid plaque size,thickness and intima-media thickness of the observation group after the treatment were lower than those of the control group,the difference was statistically significant(P〈0.05).Conclusion:Atorvastatin was effective in the treatment of acute cerebral infarction,which could reduce the level of inflammatory factors and MMP-9 and improve the cerebrovascular responsiveness and the degree of carotid artery plaque.
