用椎旁肌间隙入路手术治疗胸腰椎骨折的效果探讨

The effect of paraspinal muscle approach in thoracolumbar fracture surgery

目的 :探讨用椎旁肌间隙入路手术治疗胸腰椎骨折的效果。方法 :选取四川省成都市龙泉驿区第二人民医院2015年8月至2017年8月期间收治的82例胸腰椎骨折患者作为研究对象。对其临床资料进行回顾性分析。根据手术方法的不同将其分为观察组和对照组,每组各41例患者。为观察组患者采用椎旁肌间隙入路的手术进行治疗,为对照组患者采用后正中入路的手术进行治疗。然后比较两组患者的各项手术指标。结果 :观察组患者手术的时间、术毕至开始进行腰背肌训练的时间、术毕至下床活动的时间均短于对照组患者,其术中的出血量少于对照组患者(P<0.05)。在术后第1周,观察组患者的VAS评分低于对照组患者(P<0.05)。结论 :用椎旁肌间隙入路的手术治疗胸腰椎骨折具有创伤小、患者术中的出血量少、术后恢复快等优点。

Objective： To investigate the effect of paraspinal muscle approach in thoracolumbar fracture surgery. Methods： Select 82 cases of thoracolumbar fracture treated in The Second People＇ s Hospital of Longquanyi District during August 2015 to August 2017 for research object. Retrospectively analyzes their clinical material. According different operation methods, divide the cases to experimental group and control group,41 cases each group. Treat experimental group with the paraspinal muscle approach surgery, treat control group with posterior median approach surgery. Then compare operation indicators of 2 groups. Results： The operation time, the time of back muscle training after operation,the time of ambulation of experimental group are shorter than that in control group, bleeding volume in operation is less than that in control group （P 〈 0.05）. In a week after surgery, VAS score of experimental group is lower than that in control group （ P 〈 0.05 ） . Conclusion： Paraspinal muscle approach in thoracolumbar fracture surgery have adventures of smaller wound, less bleeding volume and shorter recovery process.
