胎盘生长因子在非小细胞肺癌胸腔积液中的临床价值

Clinical value of placental growth factor in pleural effusion of non-small cell lung cancer

目的探讨检测胎盘生长因子(PIGF)在非小细胞肺癌胸腔积液中的临床价值。方法选择在我医院就诊的非小细胞肺癌合并胸腔积液的患者117例的资料进行分析,对PIGF在非小细胞肺癌胸腔积液中的表达及其对临床疗效的影响进行探讨。结果 117例患者中,按照PIGF表达结果分为PIFG阳性组51名,PIFG阴性组66名,PIFG阴性组患者的胸腔积液治疗效果优于PIFG阳性组,PIFG阴性组患者的微血管密度值低于PIFG阳性组患者,PIFG阳性组患者的无进展生存时间与总生存时间均低于PIFG阴性组患者(P<0.05),年龄,TNM分期,淋巴转移,手术切除和PIGF表达是否阳性均为非小细胞肺癌并发胸腔积液患者死亡的独立危险因素(P<0.05)。结论对于胎盘生长因子表达阳性的非小细胞肺癌患者,其胸腔积液的治疗效果相对较差,而胎盘生长因子表达阳性也是非小细胞肺癌患者治疗效果的独立影响因素,可以作为精准医疗的一个新靶点进行开发。

Objective To investigate the clinical value of placental growth factor（ PIGF） detection in pleural effusion of non-small cell lung cancer. Methods The data of 117 patients with non small cell lung cancer complicated with pleural effusion in our hospital were analyzed,and the expression of PIGF in pleural effusion of nonsmall cell lung cancer and its influence on clinical efficacy were investigated. Results Among the 117 patients,51 cases were PIFG-positive,and 66 cases were PIFG-negative. The clinical effiecacy of the PIFG-negative group was better than that of the PIFG-positive group. The microvessel density of PIFG-negative patients was lower than that of PIFG-positive patients. The progression-free survival time and total survival time of the PIFG-positive group were lower than those of the PIFG-negative group（ P〈0. 05）. Age,TNM stage,lymph node metastasis,surgical resection and positive expression of PIGF were independent risk factors for non-small cell lung cancer complicated with pleural effusion（ P〈0. 05）. Conclusion The treatment effect of pleural effusion is relatively poor for positive placenta growth factor expression in non-small cell lung cancer patients,and the independent risk factor is positive placenta growth factor expression in treatment of patients with non-small cell lung cancer,which can also be used as a new target for the development of precision in medical care.
