经支气管镜大容量肺泡灌洗对粉尘吸入性肺病患者临床症状、肺功能及血气指标的影响

Influences of bronchoscopic high capacity lavage on clinical symptoms, lung function and arterial blood gas indexes of patients with pneumoconiosis

目的探讨经支气管镜大容量肺泡灌洗对粉尘吸入性肺病患者临床症状、肺功能及动脉血气指标的影响。方法选取我院收治的80例粉尘吸入性肺病患者,依据随机数字表法分为经支气管镜大容量肺泡灌洗术组(改良组,n=40)和传统支气管肺泡灌洗术组(传统组,n=40)。分析两组患者的临床症状、肺功能及动脉血气指标。结果灌洗后,改良组患者的咳嗽、咳痰、胸闷、呼吸困难发生率均显著低于传统组(P<0.05),改良组的FVC及FEV1%均显著高于传统组和灌洗前(P<0.05),但两组灌洗前后的pH值、Pa O_2、Pa CO_2及SPO_2比较,差异均不显著(P>0.05)。结论经支气管镜大容量肺泡灌洗较传统支气管肺泡灌洗更能有效改善粉尘吸入性肺病患者的临床症状、肺功能,且不会在较大程度上影响患者的动脉血气。

Objective To investigate the influences of bronchoscopic high capacity lavage on clinical symptoms, lung function and arterial blood gas indexes of patients with pneumoconiosis. Methods＇ Eighty patients with pneumoconiosis treated in our hospital were selected and divided into bronchoscopic high capacity lavage group （modified group, n=40） and traditional bronchoalveolar lavage group （conventional group, n=40） according to the random number table method. The clinical symptoms, lung function and arterial blood gas indexes of the two groups were statistically analyzed. Results After lavage, the incidences of cough, sputum, chest tightness and dyspnea in the modified group were significantly lower than those in the conventional group （P〈0.05）; the FVC and FEV1% in the modified group were significantly higher than those in the conventional group and before lavage （P〈0.05）; but the differences of pH, PaO2, PaCO2 and SPO2 before and after lavage of the two groups were not significant （P〉0.05）. Conclusion Compared with traditional bronchoalveolar lavage, bron- choscopic high capacity lavage is more effective in improving the clinical symptoms and lung function of patients with pneumoconiosis, and will not affect the patients＂ arterial blood gas to a larger extent.
