灰阶B超与彩色多普勒超声对胎儿脐带绕颈诊断的准确性及其对围生儿预后的影响 被引量:9

摘要 目的探讨灰阶B超与彩色多普勒超声对胎儿脐带绕颈诊断的准确性及其对围生儿预后的影响。方法回顾性分析我院192例分娩前疑脐带绕颈的晚期孕妇的临床资料,产前均经灰阶B超与彩色多普勒超声检查,并与分娩时结果进行对比;分析脐带绕颈与围生儿预后的关系。结果彩色多普勒超声诊断脐带绕颈灵敏度、准确度、阴性预测值、一致性均优于灰阶B超(P<0.05)。松绕型、紧绕型、过紧型脐带绕颈的羊水污染、胎儿窘迫发生率呈升高趋势(P<0.05)。结论彩色多普勒超声诊断脐带绕颈的准确性、一致性优于灰阶B超;两种超声可用于诊断脐带绕颈松紧程度,其对评估围生儿预后有重要意义。 Objective To investigate the accuracy of gray scale B-ultrasonography and color Doppler ultrasound in the diagnosis of fetal umbilical cord around neck and its influence on prognosis of perinatal infants. Methods The clinical data of 192 cases of late pregnant women suspected of umbilical cord around neck before delivery were retrospectively analyzed. All the wornen were examined by gray scale B-ultrasound and color Doppler ultrasound before delivery, and the results were compared with the results of delivery. The relationship between umbilical cord around neck and perinatal prognosis were analyzed. Results The sensitivity, accuracy, negative predictive value and consistency of color Doppler ultrasound in the diagnosis of umbilical cord around neck were better than those of gray scale B ultrasound (P〈0.05). The incidence of fetal distress and fetal distress caused by loose, tight, tight amniotic fluid showed an upward trend (P〈0.05). Conclusion The accuracy arid consistency of color Doppler ultrasound in the diagnosis of umbilical cord around neck are better than gray scale B ultrasound. Two kinds of ultrasound can be used to diagnose the tightness of umbilical cord around neck, which is important to evaluate the prognosis.
