摘要
目的探讨胃癌中肿瘤相关巨噬细胞(tumor-associated macrophages,TAMs)促进上皮-间质转化(epithelial-mesenchymal transition,EMT)的相关机制。方法采用免疫组化法检测90例胃癌组织和20例癌旁正常组织中TAMs、TGFβ1及EMT标志物(E-cadherin、vimentin)的表达,并分析其相关性。结果 TAMs和TGFβ1在胃癌中的阳性率均高于正常胃黏膜(P<0.05),且均与Lauren分型、分化程度、浸润深度、临床分期、淋巴结转移相关(P均<0.05);胃癌中E-cadherin阳性率低于正常胃黏膜而vimentin阳性率高于正常胃黏膜(P均<0.001)当胃癌分化程度为低~未分化、浸润深度达浆膜、临床分期达Ⅲ+Ⅳ期及有淋巴结转移时,E-cadherin表达减少而vimentin表达增加。胃癌中TAMs与TGFβ1表达呈正相关,与E-cadherin表达呈负相关,与vimentin表达呈正相关。结论 TAMs可能通过调节TGFβ1促进胃癌EMT,进而促进肿瘤浸润和转移。
Purpose To study the mechanism of tumor-associated macrophages(TAMs) promoting epithelial-mesenchymal transition( EMT) in gastric cancer. Methods TAMs markers CD68, TGFβ1, EMT-related proteins(E-cadherin, vimentin) were detected in 90 cases of gastric cancer and 20 cases of normal gastric tissue by immunohistochemistry and their relationship were evaluated. Results The expression of TAMs and TGFβ1 was significantly higher in gastric cancer than that in normal gastric tissue(P〈0. 05). The expression of TAMs and TGFβ1 was related to Lauren classification,differentiation degree, depth of invasion, clinical stage and lymph node metastasis(P〈0. 05). The present study showed a low expression of E-cadherin, but a high expression of vimentin in gastric cancer tissues. The low expression of E-cadherin and high expression of vimentin had statistically significant correlation with differentiation degree, depth of invasion, clinical stage and lymph node metastasis( P〈0. 001). Both TAMs and TGFβ1 were negatively associated with E-cadherin but positively with vimentin, TAMs was positively associated with TGFβ1 in gastric cancer. Conclusion TAMs may promote gastric cancer cells invasion and metastasis via TGFβ1-induced EMT.
