十堰地区孕妇抑郁症状调查及相关风险因素

Depressive Symptoms of Pregnant Women in Shiyan Area and Risk Factors

目的：调查分析十堰地区孕妇抑郁症状及影响的相关因素,为该地区孕产妇的心理保健提供依据。方法：采用流行病学研究中心抑郁量表（CES-D）对十堰地区妊娠期女性进行为期5年的调查与评价,分析十堰市妊娠期女性的心理健康异常的相关影响因素。结果：共380例产妇进行了心理健康状况调查分析,所有孕妇可能存在抑郁症状的共有84人,总阳性率为22.1%,其中健康组的稳定职业、家庭人均收入≥3000、医保或公费支付、产次≥1次和孕前保健的比例显著高于抑郁组（P〈0.05）,而抑郁组孕妇的有流产经历比例显著高于健康组（P〈0.05）,其中家庭人均收入（OR：3.234,95%CI：1.422～4.245）、产次（OR：2.134,95%CI：1.022～3.523）、流产经历（OR：2.342,95%CI：1.677～4.231）、孕前保健（OR：4.562,95%CI：1.345～7.234）均为孕妇合并抑郁症状的显著影响因素。结论：十堰地区的孕妇普遍存在抑郁情绪的症状,不同孕妇的家庭人均收入、产次、流产经历、孕前保健是影响孕妇合并抑郁症状的主要因素。

Objective：To investigate and analyze depressive symptoms and the influencing factors in pregnant women in Shiyan area,and to provide the basis for the mental health care of pregnant women in the area.Methods：The Epidemiology Research Center Depression Scale（CES-D）were used to investigate and evaluate the pregnant women in Shiyan area for five years.The influencing factors of mental health abnormality in women were analyzed.Results：A total of 380 delivery women attended the investigation.A total of 84 patients might have depressive symptoms,with a total positive rate of 22.1%.The percentage of delivery women with stable occupation,per capita family income≥ 3000,medicare or public payments,production≥1 times and prenatal care was significantly higher in non-depression group than that of delivery women with depression（P〈0.05）.The proportion of pregnant women with abortion in depression group was significantly higher than that in healthy group.Per capita income（OR：3.234,95%CI：1.422～4.245）,parity（OR：2.134,95% CI：1.022～3.523）,abortion experience（OR：2.342,95% CI：1.677～4.231）;health care（OR：4.562,95% CI：1.345～7.234）were significant influencing factors for depressive symptoms in pregnant women.Conclusion：In Shiyan area,the pregnant women usually have depression symptoms;per capita income,parity,abortion experience,health care were significant influencing factors for pregnant women with depressive symptoms.
