温阳活血利水方治疗急性心力衰竭阳虚水泛证25例

Twenty-Five Cases of Acute Heart Failure with Edema due to Yang Deficiency Treated with Decoction for Warming Yang and Activating Blood Circulation and Promoting Diuresis

目的:观察温阳活血利水方治疗急性心力衰竭阳虚水泛证的临床疗效。方法:将50例急性心力衰竭患者随机分成对照组和治疗组,每组各25例。对照组给予常规西医治疗,治疗组在对照组的基础上加用温阳活血利水方药。比较两组患者的临床疗效,检测两组患者治疗前后N末端钠尿肽前体(brain natriuretic titanium precursor detection,NT-ProBNP)含量;比较两组患者吗啡、呋塞米、去乙酰毛花苷、多巴酚丁胺使用量,无创呼吸机使用时间及行床旁血滤情况。结果:治疗组有效率为84.00%,对照组有效率为64.00%,两组患者临床疗效比较,差异具有统计学意义(P<0.05)。两组患者治疗后NT-Pro-BNP均低于治疗前,且治疗组低于对照组,差异具有统计学意义(P<0.05)。治疗组较对照组吗啡、呋塞米、去乙酰毛花苷、多巴酚丁胺使用量均明显减少,差异具有统计学意义(P<0.05)。治疗组较对照组无创呼吸机使用时间长,差异具有统计学意义(P<0.05)。结论:温阳活血利水方治疗急性心力衰竭阳虚水泛证疗效确切,可减少西药的用量,改善患者心功能。

Objective：To observe the clinical curative effect of Decoction for Warming Yang and Activating Blood Circulation and Promoting Diuresis on acute heart failure（AHF）.Methods：Fifty patients with AHF were randomly divided into the control group and the treatment group,with 25 cases in each group.The ones in the control group were treated with conventional Western medicine,and the ones in the treatment group were treated with Decoction for Warming Yang and Activating Blood Circulation and Promoting Diuresis.Then the clinical efficacies of two groups were compared.The contents of brain natriuretic titanium precursor detection（NT-pro BNP） of the two groups before and after the treatment were detected.The amount of morphine,furosemide,desacetyldigilanide,dobutamine dosage,noninvasive ventilation time and the use of bedside blood filter number of the two groups were compared.Results：The effective rate was 84% in the treatment group and 64% in the control group.The difference between the two groups was statistically significant（P〈0.05）.After the treatment,the two groups＇ NT-Pro-BNPs were all lower than those before the treatment,and the treatment group was lower than the control group,the difference was statistically significant（P〈0.05）.The amounts of morphine,furosemide,desacetyldigilanide and dobutamine dosage in the treatment group were lower than the control group（P〈0.05）.And all the differences were statistically significant（P〈0.05）.The use time of non invasive ventilator of the treatment group was longer than that of the control group,and the difference was statistically significant（P〈0.05）.Conclusion：Decoction for Warming Yang and Activating Blood Circulation and Promoting Diuresis has a remarkable clinical curative effect on AHF,can reduce the dosage of Western medicine and improve the cardiac function of the patients.
