养血清脑颗粒对急性缺血性脑卒中血液流变学指标及神经功能恢复的影响

Effect of Yangxue Qingnao Granule on Hemorheology and Recovery of Nerve Function in Acute Ischemic Stroke

目的观察养血清脑颗粒对急性缺血性脑卒中患者血液流变学指标的影响并探讨其机制。方法将134例急性缺血性脑卒中患者按入院先后顺序随机盲法分为对照组与观察组各67例,两组均接受卒中单元系统治疗,对照组静滴依达拉奉、溶栓及对症处理,观察组加服养血清脑颗粒。用TCD脑血流仪观测治疗期间(第1、2、3、4周)局部脑血流(rCBF),采用免疫吸附法检测脑积液中乙酰胆碱(ACH)及全血S100B含量;比色法检测血清胆碱脂酶(AChE)含量;并观察养血清脑颗粒对血液流变学指标、神经功能缺损(ESS)评分的影响。结果观察组第2、3、4周时ESS分值和r CBF均高于对照组(P<0.05);观察组脑积液中ACH高于对照组(P<0.05),血清AChE及静脉全血S100B、纤维蛋白原、血小板集聚率、全血低切黏度、红细胞压积、全血高切黏度低于对照组(P<0.05);观察组总有效率为73.13%,高于对照组的64.17%(P<0.05)。结论养血清脑颗粒有助于改善急性缺血性脑卒中患者血液流变学指标,在一定程度上可促进患者神经功能的恢复。

Objective： To observe the effect of Yangxue Qingnao Granule on Hemorheology in patients with a- cute ischemic stroke. Methods： 134 patients with acute ischemic stroke were divided into the control group and the observation group according to the random blind method of admission. The two groups were treated with stroke unit system,and the control group was treated with edaravone,thrombolytic and symptomatic treatment,and the observation group was treated with Yangxue Qingnao Granule During the treatment （the first,second,third and 4 week） ,brain flowmetry regional cerebral blood flow （rCBF）was observed with TCD. Acetylcholine （ACH） in cerebrospinal fluid and blood S100B content were detected with immunosorbent assay,serum AChE level with colorimetric method. The effect of Yangxue Qingnao Granule on hemorheology,nerve function defect （ESS） score and clinical effects were observed. Results： Compared with the control group,in second,third and 4 week,ESS scores and rCBFof the observation group were higher than those of the control group （P〈 0.05） ;ACH in brain ef- fusion of the observation group was higher than that of the control group （P〈 0.05）;serum AChE,blood S100B, fibrinogen,platelet aggregation rate and low blood viscosity,red blood cell hematocrit and whole blood viscosity were lower than those of the control group （P〈 0.05）. The efficiency of the observation group was higher than that of the control group（P〈 0.05）. Conclusion： Yangxue Qingnao Granule can improve the hemorheology of patients with acute ischemic stroke and promote the recovery of nerve function to some extent.
