文章介绍张硕甫治疗噎膈反胃的临证经验。张氏认为,噎膈反胃病位在胃,病机总属中虚气逆,临床喜用旋覆代赭汤,并体会到若不建中而纯用降气,则虚者益虚,于病无益,强调补益中焦的重要性。具体治疗上,根据疾病的不同证候,处以相应治法。或滋阴养血,和胃通幽;或通阳散结,开上润下;或温中化饮,启膈开关。法随证立,方依法出,灵活变通,并附验案,予以佐证,堪为借鉴。

The paper introduces ZHANG Shuofu＇s clinical experience in treating cardiac spasm and regurgita- tion. ZHANG considers that the location of cardiac spasm and regurgitation is stomach and the pathogenesis is middle-jiao deficiency and counter-flowing Qi. He prefers Xuanfu Daizhe Decoction,and realizs that only lower- ing Qi without tonifying middle-jiao may cause much more deficiency, so he emphasizes tonifying middle-jiao. Specific treatment according to the different syndrome of disease could be nourishing Yin and blood,harmonizing the stomach,activating Yang,removing obstruction,warming middle-jiao and resolving retained fluid. The paper also accompanies medical records to be evidence which could be learned by scholars of younger age.
