中药洗剂治疗小儿湿疹随机对照试验的Meta及聚类分析

Meta and Cluster of Randomized Controlled Trials of Traditional Chinese Medicine Lotion in Treatment of Infantile Eczema

目的系统评价中药洗剂治疗小儿湿疹的有效性与安全性,对药物组成聚类分析,为临床合理用药、制剂提供依据。方法计算机检索CNKI、VIP、Wan Fang数据库,时间为从建库至2017年10月,收集所有中药洗剂治疗小儿湿疹的RCT研究,由两名评价者按照标准对研究的质量进行评分,采用Rev Man5.2进行Meta分析。后对中药洗剂组成进行聚类分析。结果本研究共纳入21个RCT,2 280名患者。结果表明湿疹痊愈率、总有效率、复发率分别为(RR=1.57,95%CI 1.42～1.74,P<0.000 01)、(RR=1.14,95%CI 1.08～1.21,P<0.000 01)、(RR=0.45,95%CI 0.37～0.55,P<0.000 01)。聚类分析中可分为五大类。主成分因子贡献率分别为18.40%、14.89%、12.73%、10.51%、9.94%和8.55%,综合得分为-0.122～0.146。结论中药洗剂治疗小儿湿疹安全、有效、治愈率高,聚类分析为洗剂制备的选材提供了科学依据。

Objective To systematically evaluatate effectiveness and safety of Chinese herbal lotion in the treatment of infantile eczema, and cluster analysis of traditional Chinese medicine lotion composition, to provided reliable basis of rational drug use and preparation of traditional Chinese medicine lotion. Methods The databases CNKI, VIP, Wan Fang data are retrievaled by computer by the end of October 2017. Collected the randomized controlled trials（RCTs） on traditional Chinese medicine lotion in treatment of infantile eczema. Two valuators independently filtered literature assessed the methodological quality of included studies. Then meta analysis is performed using Rev Man 5.2 software. Then, Cluster analysis of Chinese medicine lotion. Results A total of 21 RCTs which involving 2 280 patients are enrolled. The results of meta analysis of the efficacy and safety showed hat the evaluation of cure rate and result of overall efficiency evaluation and evaluation of relapse rate after treatment is（RR = 1.57, 95 % CI 1.42 ~ 1.74, P〈0.000 01）.（RR = 1.14, 95 % CI 1.08 ~ 1.21, P 〈0.000 01）.（RR = 1.14, 95 % CI 1.08 ~ 1.21, P 〈0.000 01）. Chinese medicines could be clustered into 5 kinds, The contribution rate of principal component factors influence on Traditional Chinese medicine evaluation is 18.40 %、14.89 %、12.73 %、10.51 %、9.94 % and 8.55 %. Accounting for 75.01 % of the total contribution rate. and Composite score is-0.122 ~ 0.146. Conclusion The study shows that traditional Chinese medicine lotion has the advantages of safe and effective, high cure rate and low recurrence rate in treatment of infantile eczema.Cluster analysis provides a scientific basis for the selection of pediatric eczema lotion preparation.
