自锁托槽非拔牙矫治对牙弓及牙周状况的影响 被引量:15

摘要 目的探讨自锁托槽非拔牙矫治与传统托槽对牙弓及牙周状况的长期影响。方法选取2014年1月~2017年2月期间我科收治的40例牙列中度拥挤患者,分为两组,每组20名,对照组采用普通MBT托槽治疗,观察组采用Damon Q自锁托槽非拔牙矫治治疗。比较两组患者治疗前后、治疗后保持1年的牙弓、软硬组织、牙龈指数、牙周菌斑指数变化情况。结果观察组患者U1-FH、OP-FH、U1-L1明显高于对照组,U1-NA(°)、U1-NA(mm)、L1-NB(mm)明显低于对照组(P<0.05)。观察组患者ULP、N'-Sn-Pg'明显低于对照组,Sn Ls-Si Li、Cm-Sn-Ls、Ls-E、Li-E明显高于对照组(P<0.05)。观察组患者T2、T3期牙周菌斑指数明显低于对照组(P<0.05)。观察组患者T2、T3期牙龈指数明显低于对照组(P<0.05)。结论 Damon自锁矫治器非拔牙矫治牙列中度拥挤长期疗效较稳定,在控制菌斑及牙龈出血方面有优势。 Objective To investigate the long-term effects of non extraction self ligating brackets and traditional brackets on the arch and periodontal condition. Methods A total of 40 patients were included in this study. The patients were divided into two groups of 20,the control group used the MBT bracket treatment,and the observation group was treated with Damon Q self ligating bracket with non extraction treatment. The changes of arch,soft and hard tissues,gingival index and plaque index of teeth were compared between the two groups before and after treatment. Results The levels of U1-FH,OP-FH and U1-L1 in the observation group were significantly higher than those in the control group,and the U1-NA( degree),U1-NA( mm) and L1-NB( mm) were significantly lower than those of the control group( P 〈0. 05). ULP and N ’-Sn-Pg’ in the observation group were significantly lower than those in the control group,and Sn LsSi Li,Cm-Sn-Ls,Ls-E,Li-E significantly higher than in the control group( P 〈 0. 05). The plaque index of T2 and T3 in the observation group was significantly lower than that in the control group( P 〈 0. 05). The gingival index of T2 and T3 in the observation group was lower than that in the control group( P 〈 0. 05). Conclusion Self ligating orthodontic appliance is an effective method for the treatment of moderately crowded dentition with long-term stability,and has advantages in the control of plaque and gingival bleeding.
