颈肩肌群按揉牵拉辅治小儿斜颈的应用效果及对预后影响分析

EFFECT OF NECK AND SHOULDER MUSCLES MASSAGE TRACTION IN THE AUXILIARY TREATMENT OF PEDIATRIC TORTICOLLIS AND ITS INFLUENCE ON PROGNOSIS

目的分析颈肩肌群按揉牵拉辅治小儿斜颈的应用效果及预后影响,为临床治疗提供有效依据。方法选择2015年4月—2016年8月该院儿科收治的肌性斜颈患儿60例,将其按便利法分为观察组(n=30)和对照组(n=30),对照组采用传统推拿手法,每次30min,1次/d,1疗程/周。观察组在对照组的基础上给予颈肩肌群按揉牵拉治疗,观察比较二组患儿的治疗效果、超声波彩超下治疗前后肿块厚度及患儿接受治疗时间早晚对治疗程度的影响。结果观察组的总有效率为96.67%,对照组为80.00%,观察组患儿有效率显著高于对照组患儿,二者比较具有统计学意义(χ~2=2.44,P>0.05)。观察组治愈效率为90.00%,对照组为60.00%,二者比较具有统计学意义(χ~2=3.05,P<0.05)。超声显示二组患儿经治疗后肿块直径均较治疗前缩短,观察组肿块缩小程度明显大于对照组,且治疗时间越长差异越大,二组患儿比较差异具有统计学意义(P<0.05)。患儿越早接受治疗,患儿在治疗时效果越明显,年龄小于2个月患儿治疗总有效率高于其他年龄段组,比较具有统计学意义(P<0.05)。结论颈肩肌群按揉牵拉辅治小儿斜颈在临床上有较好临床疗效,值得临床推广。

Objective To analyze the application effect and prognosis of the neck shoulder muscle group by kneading traction in children with torticollis,and provide effective basis for clinical treatment.Methods Sixty cases of muscular torticollis in author＇s department were divided into observation group（n=30）and control group（n=30）according to random number table method.The control group was treated with traditional massage technique for 30 minutes each time,while the observation group was treated with the neck shoulder muscles massage traction treatment based on the control group.Then the treatment effect of the two groups was observed and compared using the thickness of the mass by ultrasonic color Doppler ultrasound before and after treatment and the time of treatment of children.Results The total effective rate of the observation group was 96.67%,while that of the control group was 80.00 %（χ2=2.44,P〉0.05）.The cure rate was 90.00 %in the observation group and 60.00 %in the control group（χ2=3.05,P〈0.05）.Ultrasound showed that the diameter of the tumor in the two groups after treatment was shorter than before treatment,the size of the observation group was significantly smaller than the control group,and the longer the treatment time,the difference was statistically significant（P〈0.05）.The more the early treatment,the more obvious the effect of the children in the treatment,the total effective rate of the children aged less than2 months was higher than other age groups,the difference was statistical significance（P〈 0.05）.Conclusion The neck shoulder muscle kneading and traction with the child torticollis has a better clinical effect,and it is worthy of clinical promotion.
