共情式心理干预对自体DC/CIK细胞维持治疗肺腺癌患者癌因性疲乏、希望水平的影响

Effect of empathy mental intervention on the level of hope and cancer related fatigue in the nursing of autologous DC/CIK cells maintenance in the treatment of advanced pulmonary adenocarcinoma

目的探讨共情式心理干预对自体树突状细胞(dendritic cells,DC)/细胞因子诱导的杀伤细胞(cytokine-induced killer,CIK)细胞维持治疗晚期肺腺癌患者癌因性疲乏、希望水平的影响。方法选取2015年1月—2017年3月湖州市中心医院收治的行自体DC/CIK细胞维持治疗的晚期肺腺癌患者100例,采取随机数字表法分为2组,每组50例,对照组给予常规护理,观察组在对照组护理的基础上给予共情式心理干预。比较2组患者护理前后焦虑自评量表(self-rating anxiety scale,SAS)评分、抑郁自评量表(self-rating depression scale,SDS)评分、Piper疲乏量表(piper fatigue scale,PFS)评分、Herth希望量表(herth hope index,HHI)评分以及护理满意度。结果护理后,观察组ASA评分、SDS评分均明显低于对照组,差异均有统计学意义(均P<0.05);观察组PFS总分以及认知、感觉、情感、行为评分均明显低于对照组,差异均有统计学意义(均P<0.05);观察组HHI总分以及与他人保持密切关系、采取积极行动的态度、对现实及未来态度评分均明显高于对照组,差异均有统计学意义(均P<0.05);观察组满意率为96.00%,明显高于对照组的82.00%,差异均有统计学意义(P<0.05)。结论共情式心理干预应用于自体DC/CIK细胞维持治疗的晚期肺腺癌患者,能减轻患者癌因性疲乏和负性情绪,且明显提高了患者的希望水平。

Objective To investigate the effect of empathy mental intervention on the level of hope and cancer related fa- tigue in the nursing of autologous dendritic cells （DC）/cytokine-induced killer （CIK） cells maintenance in the treatment of advanced pulmonary adenocarcinoma. Methods A total of 100 patients with advanced pulmonary adenocarcinoma who were given the treatment of autologous DC/CIK cells maintenance in Central Hospital of Huzhou from January, 2015 to March, 2017 were selected, and divided into two groups by random number table （n = 50） ： The control group received routine nursing care; the observation group was given empathy mental intervention based on empathy nursing in the con- trol group. The self-rating anxiety scale （SAS） score, self-rating depression scale （SDS） score, piper fatigue scale （PFS） score, Herth hope index （HHI） score and nursing satisfaction were compared between the two groups before and after nursing. Results After nursing, the ASA scores and SDS scores of observation group were significantly lower than those of the control group, with statistical significance （ all P 〈 O. 05 ） ; The PFS score and cognition, feeling, emotion, behavior scores of observation group were significantly lower than those of the control group （ all P 〈 O. 05 ） ; The HHI score and close relationship with others, attitudes towards taking positive action, attitude to the reality and future scores of observation group were significantly higher than those of the control group （ all P 〈 O. 05 ） ; The satisfaction rate of observation group was 96%, which was significantly higher than 82% of control group （ P 〈 0.05 ）. Conclusion Empathy mental intervention in the nursing of autologous DC/CIK cells maintenance in the treatment of advanced pulmonary adeno-carcinoma can reduce cancer-related fatigue and negative emotion, and obviously improve the patient＇ s level of hope.
