全血C-反应蛋白联合白细胞计数在儿童呼吸道感染中的价值分析

摘要 目的分析全血C-反应蛋白(CRP)联合白细胞计数(WBC)在儿童呼吸道感染中的价值。方法选取医院收治的呼吸道感染患儿260例为研究对象,分为细菌性感染组150例和病毒性感染组110例,另选择同期在我院体检的健康儿童80例作为对照组,观察3组CRP和WBC水平,治疗前后细菌性感染组儿童的全血CRP和WBC水平的变化。将CRP>8 mg/L,WBC>10×109/L作为阳性标准,计算出全血CRP与WBC诊断细菌感染的灵敏度、特异度、阳性预测值以及阴性预测值。结果细菌性感染组儿童的全血CRP和WBC水平明显高于病毒性感染组和对照组,3组儿童的全血CRP和WBC水平相比(P<0.05);治疗后,细菌性感染儿童全血CRP和WBC水平低于治疗前(P<0.01);当CRP>8 mg/L时,CRP诊断细菌性感染的灵敏性、特异性、阳性预测值、阴性预测值分别为85.24%、49.61%、55.18%、82.37%;当WBC>10×109/L时,CRP诊断细菌性感染的灵敏性、特异性、阳性预测值、阴性预测值分别为88.16%、69.42%、67.84%、87.53%。结论全血CRP联合WBC检测,对儿童呼吸道感染具有较高的价值,可以初步判断感染病原,为早期经验性抗菌药物治疗提供重要参考依据。
