磁共振动态增强扫描联合磁共振扩散加权成像在乳腺癌患者诊断中的应用价值 被引量:13

摘要 目的探讨磁共振动态增强扫描(DCE-MRI)联合磁共振扩散加权成像(DWI)在乳腺癌患者诊断中的应用价值及相关生物学行为。方法选取2015年4月至2017年4月我院就诊的74例疑似乳腺癌患者,均行DCE-MRI、DWI检查,统计两者联合诊断准确度、敏感度、特异度。结果以病理结果为金标准。74例疑似乳腺癌患者确诊34例良性,40例恶性,DCE-MRI确诊21例良性,31例恶性,DWI确诊23例良性,30例恶性,两者联合确诊32例良性,39例恶性。两者联合诊断特异度(94%)、敏感度(98%)、准确度(96%)均高于DCE-MRI、DWI单一诊断,差异有统计学意义(P<0.05)。结论 DCE-MRI联合DWI有助于提高乳腺癌患者诊断特异度、敏感度、准确度,对推动乳腺癌影像学发展、协助临床手术方案制定及预后判断具有积极意义。 Objective To investigate the application value of dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI) combined with diffusion-weighted imaging(DWI) in the diagnosis of breast cancer and the related biological behavior. Methods A total of 74 cases of suspected breast cancer patients in our hospital from April 2015 to April 2017 were examined by DCE-MRI and DWI. The diagnostic accuracy, sensitivity and specificity of the combined diagnostics of these two methods were combined. Results The pathological results were taken as the gold standard. Seventy-four cases of suspected breast cancer were diagnosed 34 cases of benign and 40 cases of malignant.DCE-MRI confirmed 21 cases of benign and 31 cases of malignant. DWI confirmed 23 cases of benign and 30 cases of malignant. The combined diagnosis confirmed 32 cases of benign and 39 cases of malignant. The specificity(94%),sensitivity(98%), and accuracy(96%) of the combined diagnosis were higher than those of the single diagnosis of DCE-MRI and DWI, and the difference was statistically significant(P〈0.05). Conclusion DCE-MRI combined with DWI can help improve the diagnostic specificity, sensitivity and accuracy of breast cancer. It has a positive significance in promoting the development of breast cancer imaging and assisting the formulation of clinical operation plan and prognosis.
