混合劈核法联合自制劈核针治疗老年性白内障

摘要 目的探讨混合劈核技术在白内障超声乳化手术中的应用效果。方法将单纯年龄相关性白内障100例(100眼)Ⅲ~Ⅳ级核患者,随机分为混合劈核组(A组)和常规水平劈核组(B组)。术中记录两组平均超声乳化能量及平均超声乳化时间,分别于术后1 d、1个月观察两组角膜水肿情况。结果 A组平均超声能量为(8.0±1.1)%,B组平均超声能量为(24±3.2)%;A组平均超声时间为(53.2±6.3)s,B组平均超声时间为(146.5±15.7)s;两组比较差异有统计学意义(P<0.01)。A组Ⅲ级硬度核术后第1天角膜水肿0眼,B组为2眼,两组比较差异有统计学意义(P<0.01);A组Ⅳ级硬度核术后第1天角膜水肿2眼,B组为8眼,两组比较差异有统计学意义(P<0.01)。结论混合劈核技术应用于白内障超声乳化手术中,可缩短术中平均超声乳化时间,减少术后角膜反应及术中并发症,有效恢复患者视力。 Objective To investigate the effect of mixed nucleus chopping technique in phacoemulsification.Methods The patients with simple age-related cataract(n=100,100 eyes) were randomly divided into two groups:mixed nuclear group(group A) and normal group(group B). The average ultrasonic emulsification energy and the mean phacoemulsification time were recorded during the operation. Corneal edema in two groups was observed at one day and one month after operation respectively. Results The average ultrasound energy in group A and group B were(8.0±1.1)% and(24.0±3.2)%,respectively;the average ultrasonic time in group A and group B were(53.2±6.3)s and(146.5±15.7)s,respectively. The difference between the two groups was statistically significant(P〈0.01). There was no corneal edema in the patients with Grade Ⅲ nuclei in group A,which were 2 eyes in group B. Corneal edema occurred in 2 eyes with Grade IV nuclei in group A,which were 8 eyes in group B. The difference between the two groups had statistical significance(P〈0.01). Conclusion The mixed chopping technique can shorten the average time of phacoemulsification,reduce the corneal reaction and complications,and improve the visual acuity in the patients with cataract.
