人血清附睾蛋白与糖类抗原检测在卵巢癌诊断中的价值

Value of serum HE4 and CA125 single and combined detection in ovarian cancer diagnosis

(1)目的探讨人血清附睾蛋白(HE4)与糖类抗原(CA125)联合检测在卵巢癌诊断和预后评估的作用和价值。(2)方法收集上海市嘉定区妇幼保健医院HE4和CA125联合检测数据55例(包括:卵巢癌组30例、卵巢良性肿瘤组25例),采用SPSS 19.0软件分析数据,评估血清HE4、CAl25在鉴别诊断卵巢良恶性肿瘤中的意义。(3)结果 HE4与CA125联合检测的灵敏度、特异度、阳性预测值、阴性预测值、约登指数均要明显高于HE4和CA125单项检测数值;晚期(Ⅲ~Ⅳ期)22例卵巢癌患者血清中HE4和CA125水平明显高于早期(Ⅰ~Ⅱ期)8例卵巢癌患者,P<0.05,差异有统计学意义;浆液性癌及子宫内膜癌的血清HE4和CA125表达高于其他几组水平。将浆液性癌、粘液性癌、子宫内膜癌及透明细胞癌每两组之间的HE4和CA125水平经过比较分析,P<0.05,有统计学意义;对相同治疗阶段18例卵巢癌患者实施卵巢癌细胞减灭术后,血清HE4与CA125中位数均处于下降趋势,呈现正向关系。(4)结论 HE4和CA125联合检测对卵巢癌的检测有重要意义;卵巢癌治疗中HE4的诊断价值要优于CA125;HE4和CA125用于诊断浆液性癌方面可能更有价值;血清HE4和CA125联合检测对于卵巢癌的分期检测具有指导意义。

Objective To explore the role and value of HE4 and CA125 in the diagnosis and prognosis of ovarian cancer.Methods To collect the Shanghai jiading district maternity and child health care hospital HE4 and CA125 testing data of 55 cases（including：ovarian cancer group 30 cases,25 cases of ovarian benign tumor group）,adopt SPSS 19.0 software to analyze data,evaluation of serum HE4 and CAl25 significance in differential diagnosis of benign and malignant ovarian tumors.ResultsThe sensitivity,specificity,positive predictive value,negative predictive value and Jordan index of the combined detection of HE4 and CA125 were significantly higher than that of HE4 and CA125.The late（Ⅲ-Ⅳ period）,22 cases of ovarian cancer patients serum HE4 and CA125 levels were significantly higher than those in the early phase（Ⅰ-Ⅱperiod）,8 cases of ovarian cancer patients（P〈0.05,the difference was statistically significant;serous carcinoma and endometrial cancer serum HE4 and CA125 expression is higher than other groups of level,will be serous carcinoma,mucinous carcinoma and endometrial carcinoma and clear cell carcinoma between every two groups of HE4 and CA125 levels through comparative analysis,P〈0.05,suggesting that have statistical significance.After the operation of ovarian cancer cells in 18 cases of ovarian cancer in the same treatment stage,serum HE4 and CA125 median were both in decline,presenting positive relationship.Conclusion HE4 and CA125 are important for the detection of ovarian cancer.The diagnostic value of HE4 in the treatment of ovarian cancer is better than that of CA125;HE4 and CA125 may be more diagnostic in diagnosis of serous carcinoma.The joint detection of serum HE4 and CA125 has guiding value for the staging of ovarian cancer.
