初治多发性骨髓瘤改良VAD方案治疗过程中系统化护理干预效果观察

Effect of systematic nursing intervention on the treatment of VAD with modified multiple myeloma

目的:探讨系统化护理干预对初治多发性骨髓瘤(MM)改良VAD方案治疗过程中的临床价值。方法:选取接受改良VAD方案治疗的50例初治MM患者,按入院顺序随机分为对照组和观察组,各25例。对照组接受常规化疗护理,观察组在对照组基础上实施系统化护理干预。对比两组患者临床疗效及跟踪随访1年的生存率和生存质量。结果:两组患者消化道反应、周围神经炎、肝功能异常、血糖血压升高发生率及跟踪随访1年的总体生存率、平均生存时间比较无明显差异,无统计学意义(P>0.05);观察组骨髓抑制和感染发生率明显低于对照组,生存质量总评分、完全缓解率、总有效率明显优于对照组,差异有统计学意义(P<0.05)。结论:初治MM患者接受改良VAD方案治疗过程中实施系统化护理干预措施,可有效促进患者病情缓解,减轻不良反应,提高生存质量。

Objective： To investigate the clinical value of systematic nursing intervention in the treatment of patients with newly diagnosed multiple myeloma （MM） modified VAD regimen. Method： Fifty patients with newly diagnosed MM who underwent modified VAD regimen were randomly divided into control group and observation group according to admission sequence. The control group received routine chemotherapy and the observation group received systematic nursing intervention on the basis of the control group. The clinical efficacy and follow-up of 1-year survival rate and quality of life were compared between the two groups.Results： There was no significant difference between the two groups in the incidence of gastrointestinal reactions, peripheral neuropathy, abnormal liver function, the incidence of elevated blood glucose and blood pressure and the follow-up 1-year overall survival and mean survival time （ P 〉 0.05） The incidence of bone marrow suppression and infection were significantly lower in the control group. The total quality of life score, complete remission rate and total effective rate were significantly better than those in the control group （ P 〈 0.05）.Conclusion： Patients with newly diagnosed MM undergo systematic nursing interventions during the course of treatment with the modified VAD regimen, which can effectively improve the patient＇s condition, reduce adverse reactions, and improve the quality of life.
