摘要
将42颗患种植体周围炎(PI)的植体随机分为3组:机械清创(MD)组(A)、MD+光动力疗法(PDT)组(B)、MD+盐酸米诺环素软膏(MHO)组(C)。于基线(T0)、治疗后6周(T1)及12周(T2)检测探诊深度(PD)、改良出血指数(m SBI)、改良菌斑指数(m PLI)及龈沟液(PICF)内白介素-1β(IL-1β)及肿瘤坏死因子-α(TNF-α)的水平。发现3组T1、T2时的临床指标及T2时PICF内IL-1β、TNF-α水平均较T0时显著降低(P<0.05);除m PLI外,治疗后各时间点B、C组的各项指标均显著低于A组(P<0.05),但B、C组间差异无统计学意义。表明相比于单纯MD,PDT辅助治疗PI,可进一步改善临床指标,降低PICF内炎症因子水平,效果与局部应用MHO相当。
42 implants diagnosed as peri-implantitis( PI) were randomly divided into 3 groups: mechanical debridement( MD)( group A),MD + photodynamic therapy( PDT)( group B),MD + minocycline hydrochloride ointment( MHO)( group C). At baseline( T0),6 weeks( T1) and 12 weeks( T2) after treatment,probing depth( PD),modified bleeding index( m SBI),modified plaque index( m PLI) and the level of IL-1β and TNF-α in peri-implantal crevicular fluid( PICF) were detected. The clinical parameters at T1 and T2 and the level of IL-1βand TNF-α in PICF at T2 in all 3 groups were significantly lower than T0( P〈 0. 05); In addition to m PLI,at each time point after treatment,all parameters of B and C group were significantly lower than A group( P〈0. 05),but there was no statistical difference between B and C group. MD with adjunctive PDT to treatment of PI can further improve the clinical parameters and reduce the level of inflammatory cytokines in PICF. The effect is similar to that of topical application of MHO.
